Found that the E&S plan had been implemented properly up to that time. Noted no accelerated erosion & sedimentation on the site at the time of the inspection. Spoke with operator representative Mr. David Miller and requested that the E&S plan continued to be implemented properly, and that reclaiming any distrubed acreage that would require no further work during site development be reclaimed and seeded and mulched. In addition the Department suggested that the operator rock the entire access road and the operator representative agreed that rocking the entire access road would be a prudent course of action and that it will be undertaken. Met with and spoke with complainant regarding the findings of the inspection.
1743794 Primary Facility 2008-10-16 Complaint Inspection No Violations Noted Inspection found that no erosion & sedimentation had occurred and that the access road was still under construction. Spoke with the complainant regarding the inspection and informed him of the findings.

1762342 Primary Facility 2009-01-14 Complaint Inspection No Violations Noted Site inspection performed as a result of complaint received regarding the pit. Drilling had been completed at the time of the inspection. The site had not as of yet been regraded. Found no accelerated erosion & sedimentation on the site or access road at the time of the inspection. Noted a below grade pit still located onsite. The pit had more than two feet of required freeboard, in addition noted that the liner appeared to be intact and in sound condition. The operator should continue to monitor the pit to insure its continued integrity in addition to monitoring the site and access road for accelerated erosion & sedimentation until final restoration activities are completed and the site is permantly stabilized.

Spoke with operator representative Dave Miller regarding dressing up the access road entrance directly off the township road and monitoring of the culvert carrying the existing drainage ditch underneath the access road. The operator should continue to monitor the well-site and access road for accelerated erosion & sedimentation until final restoration activities are completed.
