Brine tank located onsite and appeared to be in sound condition. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. In addition noted that the well had been located in a farm field, which at the time of the pre-drill waiver inspection was a planted corn field. Found no well tag on any of the well apparatus located onsite. The Department requests that the operator forward documentation showing the well tag has been affixed no later than 08/17/09 & a completed Well Record & Completion report, to the attention of Gregory Higgins at the above address.
