The Department notified operator representative Gordon Bish by phone of the below-noted observations. The Department is requesting the following information at this time: The last time the brine tank was inspected, the last time the tank was emptied, and the tank readings from the last time the tank was emptied through today.

Mr. Bish stated that the tank is inspected weekly. The operator will promptly investigate and follow-up with the Department.

The operator appears to be in compliance with policy at this time. Violations may be forthcoming pending operator response and investigation.

2705449 Primary Facility 2018-03-13 Administrative/File Review Violation(s) Noted Admin inspection conducted in response to inspection record 2702279 in which the Department identified a possible release of production fluids to the ground.

Operator representative Gordon Bish contacted the Department by phone. Upon further investigation, the operator determined the following: The well is inspected on a weekly basis, sometimes as often as three times a week because of its substantial gas production. The well does not produce water unless the well is blown. The well had been blown and on September 20, 2017, a pinhole was discovered in the brine tank where brine was actively spurting out of the hole. To stop the discharge, the screw (observed in inspection record 2702279) was placed within the pinhole. Before 9/20/17, the tank measurement was 112 inches. After the leak was discovered, 111.15 inches of brine (equating to 95 bbl of fluid) was hauled out from the tank, indicating that 0.85 inches of fluid was lost. Operator representative Lisa Raffle notified the Department by phone that remediation activities, scheduled to commence tomorrow, may be postponed pending the forecasted weather conditions. Remediation activities, specifically delineation of the impacted area, will commence upon improved weather conditions.

The October 3, 2014 Program Clarification Memo was attached to this email for guidance.
