Open lined earthen pit was sitting nearly full of water with liner submerged in places. Several pieces of equipment were staged on location with license plates, registration and inspection stickers that had expired several years ago. Vegetation had grown up around the equipment.
2499039 Primary Facility 2016-06-27 Compliance Evaluation Outstanding Violations - No Viols Req'd Spoke with operator regarding when well will be equipped for production or plugged and excess equipment removed to correct violations. I was informed that operator does not have any immediate plans to correct violations and may be filing for bankruptcy shortly. Immediately reported this to SWRO.

2734804 Primary Facility 2018-05-29 Compliance Evaluation Outstanding Violations - Viols Req'd This well is tagged and in production. This was a follow up compliance evaluation inspection. I noted that only partial compliance has been achieved on this well pad as two trucks, a ditcher and a trailer load of well tubing has been removed from the well pad but the service rig remains set up over the well and the rig doghouse, large fluid collection tank and various other debris which was all to be removed still remains on site. It is my finding therefore that this well site is in outstanding and continuing violation status.

2831313 Primary Facility 2019-01-23 Follow-up Inspection In Compliance with Schedule An Order (Enforcement #362085) was issued against this well on 3-5-18.

At time of inspection access road and site are snow-covered. At this time well appears equipped for production. Well tag present (photo #1). Service rig in place over well head (photo #2). Rig doghouse, compressor, tank, and various other pieces of equipment are on site (photo #3). Violations issued by DEP OGI on 12-7-15 remain outstanding.
