2749425 Primary Facility 2018-07-06 Administrative/File Review In Compliance with Policy Admin inspection in response to reported release of 25 gallons production water onto well pad.

Department spoke to operator representative Derek Zofchak by phone: A leak occurred on a hammer union of the 2” dump line located approximately 12 inches from the #11 GPU, which resulted in a release of 25 gallons production water to the graveled well pad. It is unknown how long the leak was occurring; however, personnel are at the site on a daily basis. The quantity released was estimated based on the area of impact, the size of the line, and the frequency in which produced water flows through the line (which is intermittently). Mr. Zofchak is currently on-site with the contractor. There are two roll-off boxes on-site; however, only one is anticipated to be necessary. The #11 GPU is currently being rigged down for temporary removal. As soon as the #11 GPU is removed, excavation of the contaminated materials will occur.

2750077 Primary Facility 2018-07-09 Follow-up Inspection Violation(s) Noted Follow up to a release reported by the Operator to the Department on 7/6/2018, see inspection report 2749425. At the time of inspection, GPU 11 appeared to be equipped for production. The Department notes that the GPU 11 is not placed in secondary containment. GPU 11 was installed as part of the production equipment associated with 003-22465, spud on 10/31/2017.

The Department observed new gravel under and beside GPU 11. The Department also observed a black covered red container on site. The Operator appears to have removed the visually impacted material from the well pad. The Department requests copies of the waste manifests related to the disposal of the impacted soil. The Department also spoke with Operator representative Dennis Furmanek regarding the release and absence of secondary containment. Based on the observations made during this inspection the Department is issuing violations. The Department recommends that the Operator equip the GPU with secondary containment.
