2400635 Primary Facility 2015-08-07 Routine/Complete Inspection No Violations Noted On 8/7/15, I inspected the Bovard Dorothy site as a routine inspection and made the following observations:
2454927 Primary Facility 2016-02-25 Routine/Complete Inspection No Violations Noted THE WELL, TANKS, AND PRODUCTION EQUIPTMENT HAVE BEEN WELL MAINTAINED.

THE PAD AND ACCESS ARE IN GOOD CONDITION.

2504524 Primary Facility 2016-04-07 Routine/Partial Inspection No Violations Noted On 4/7/16, I performed a routine partial site inspection at the Dorothy Bovard site in response to a request from the operator and met with Range Resources representative Erica Taylor to review requested alteration to the shape of the pad post restoration. The change was approved as a redline change as it did not appear to significantly change flow around the site and decreased impervious surface area slightly.
2566356 Primary Facility 2016-11-16 Complaint Inspection No Violations Noted On 11/16/16, I performed a complaint inspection related to the Bovard site and made the following observations:

•The complainant reported that beginning in late September 2016 their well began producing gray water. Within a few days the water changed to black with intermittent gray at which time the complainant called their well service company to inspect with inconclusive results. At this time the floss filter was reported to have begun clogging regularly and a rotten egg odor was noted. •The complainant reported being in residence for 46-48 years. The well bore is located roughly 10 feet from the house and had a secure cap elevated 6” off of the ground at the time of inspection. The complainant reported that the well was drilled to 240 feet deep in 1988 and has a poly case to 220 feet. The well was reported to have had prior issues with sediment. •Samples were taken from the well for 946 and bacteriological analysis before treatment and after a ten-minute flush. •Inspection of the well site found no i

2571020 Primary Facility 2017-01-13 Administrative/File Review No Violations Noted On 1/13/17, I performed an Admin inspection related to the Bovard Dorothy site in response to complaint 323595 and made the following observations:

•Review of lab results for samples taken 11/16/17 as well as other situational information and found that, while water chemistry parameters were high in multiple instances, oil and gas impact was not conclusively indicated.

2627019 Primary Facility 2017-03-27 Complaint Inspection No Violations Noted On 3/27/2017 a COMPL inspection of the Bovard well and an associated site was completed in response to complaint 325615. The following was observed:

>>E&S controls in multiple locations around the site were observed to be unnecessary and/or in poor repair. All controls should be removed or maintained as appropriate.

>>No evidence of spill or other surface contamination was noted in relation to possible water supply impact.

>>In response to reported bacterial contamination and off taste of the complainants drinking water samples were collected for SAC 946 and bacteriological analysis.

>>The distance from the water supply to the nearest applicable oil & gas activity is roughly 4100 feet. No direct impact from the site was observed.

>>No evidence was observed at the time of inspection to indicate overt surface water impact. The water well head was found to be in good condition.

2657238 Primary Facility 2017-11-06 Site Restoration No Violations Noted Post-Drilling well site restoration report submitted on 8/11/2026 and resubmitted electronically on 4/28/2017. Site ID sign located at beginning of access road. PPC plan on site. Eight unconventional gas well drilled and appear to be equipped for production. Eight gas production units on site and appeared to be equipped for production. Four brine tanks on site. Brine tanks appear to be in sound condition. Secondary containment surrounding brine tanks appeared to be in sound condition. No sheen observed inside the secondary containment. Temporary erosion and sedimentation BMPs removed. No observation of accelerated erosion or sedimentation in relation to the well site or access road. Post-drilling well site restoration report approved.
2671059 Primary Facility 2017-09-11 Administrative/File Review No Violations Noted On 9/11/17 an administrative inspection related to the Bovard Dorothy well site was conducted in response to complaint 325615. The following was found:

>>Sample results for water chemistry and bacteria load were analyzed.

2684683 Primary Facility 2018-01-22 Routine/Complete Inspection No Violations Noted Well drilled and equipped for production. Well tag present. Well apparatus, brine tanks and gas production units appear to be in sound condition. No observation of accelerated erosion or sedimentation observed in relation to the well pad or access road.
2728981 Primary Facility 2018-05-11 Routine/Complete Inspection No Violations Noted Multi-well pad. At the time of this inspection, all 8 wells on this pad are drilled and appear equipped for production.
