The site sign was in place and properly displayed the permits. The site E&S controls and BMP's were well constructed and in good shape. The access road was constructed according to plan and E&S controls were working properly. The well was drilled. The gathering line for the site was being constructed. The BMP's were in place and hillsides in this area were temporarily stabilized.
2131905 Primary Facility 2013-01-22 Incident- Response to Accident or Event In Compliance with Policy On 10/11/12, around 22:15, a release of approx. 30-35 gal reuse water occurred. Reuse water is being hauled to the site to use for the current frac operation. A new driver for Equipment Transfer pressed the wrong control while backing up to tanks, and sent water out of a spreader bar (which should NOT have been attached) onto the ground and into the tank containment. Weavertown Environmental was contacted to remediate the area by excavating affected gravel/soil, placing it into roll-off containers for disposal. The disposal records will be sent to you upon receipt.

2483035 Primary Facility 2016-01-04 Routine/Partial Inspection No Violations Noted On 1/4/16, I performed a routine partial inspection of the Bakerstown Road Pad in response to an operator request for review of redline changes and made the following observations:

•The operator requested a redline change involving a temporary waterline crossing the site access road. The request was approved. •Site equipment appeared in good condition. Snow cover prevented inspection of surface features in most areas.

2660519 Primary Facility 2017-11-15 Site Restoration No Violations Noted Inspection in response to Post-Drilling Site Restoration Report received on 9/19/2017. Operator representative Erica Taylor also on site. ESCGP permit on site. PPC plan on site. Well pad area reduced in size. Rock removed from well pad stock piled on tank pad adjacent to well site. Operator indicated that rock will be reused on the JR Miller Well Site. The transportation of the stone is scheduled to begin on Friday 11/17/2017. Temporary E&S BMP silt sox remain on the perimeter of the reclaimed portion of the well pad. Operator indicated the silt sox is scheduled will be removed in the next few weeks. Operator also indicated that a plan to re-permit the site in order to keep the tank pads has been submitted to the Department and is under review. Earth disturbance activity authorized by the permit appear to be complete. No observation of accelerated erosion or sedimentation in relation to the well site or access road.
