1985349 Primary Facility 2011-06-09 Routine/Complete Inspection No Violations Noted On June 9th, 2011 I conducted an inspection for an OG 57 Request for waiver of distance requirement. The well site is proposed to be constructed within 100 ft. from Deer Creek. The proposed well was site located. Construction has not begun at site. Proposed E & S plan should protect stream from adverse impact during site/well development if properly implemented & maintained. If drill cuttings are to be encapsulated the bottom of the pit must be 20¿ above the seasonal high groundwater table and a property owner waiver would be necessary due to the close proximity of the building at the location.
1998293 Primary Facility 2011-08-10 Routine/Complete Inspection No Violations Noted On August 10th, 2011 I conducted a routine inspection of the above site. Drilling had started. The pit appeared to be in good condition. A site ID and E&S plan were posted on site. The access road was in good condition. The location is in the corner of an asphalt parking lot. The conductor was in place. E&S controls were constructed around the perimeter. Some tophole water was on the pad. The operator should continue to inspect the E&S controls routinely.

1998352 Primary Facility 2011-08-11 Drilling/Alteration No Violations Noted DRILLER HIT A LARGE AMOUNT OF WATER AT 172'. THE WATER STREAM WAS ARTESEAN AND WAS PUMPED INTO A FIELD AND RUN THROUGH FILTER SOCKS. WHEN THE WATER BECAME TO MUCH TO CONTAIN THE DECISION WAS MADE TO PLUG THE WELL. THE WELL WAS CEMENTED TO THE SURFACE.
1998434 Primary Facility 2011-08-11 Routine/Complete Inspection Violation(s) Noted On August 11th, 2011 I conducted an inspection of the above site in response to notification of a tophole water release. Drilling was underway. During the night, tophole water coming from the well had overwhelmed the E&S controls and was unable to be contained in the pit. Water was flowing out of the well, across the ground, through the E&S controls and into Deer Creek, classified as a CWF. The operator contacted DEP when the discharge began, approximately 1:00 am. Diligent efforts were made to stop or decrease the amount of water reaching the stream. Water was being collected in a pit and frac tank. Additional E&S controls were installed (haybales). Excess water from the pit was being discharged through the use of pumps and filter bags into a well vegetated area adjacent to the site. There was no discharge into the stream at the time of my inspection at 13:00

2002571 Primary Facility 2011-08-18 Compliance Evaluation No Violations Noted On August 18th, 2011 I conducted an inspection of the above site as a follow up to the tophole water release on 8/11/11. Drilling had been halted. The well had been plugged. The rig was moved offsite. E&S controls were adequate. The pit was still open and contained some fluids. The access was ok, a site ID and E&S plan were still posted on site.
