(Operator failed to maintain E&S controls leading to sedimentation being carried off site and around controls. The silt fences on the sides of the access road are overwhelmed with sediment and are not implemented correctly.) All E&S violations noted above have been abated and the operator is in compliance. The site also has been seeded and mulched.

1850075 Primary Facility 2009-12-14 Routine/Complete Inspection No Violations Noted The operator submitted a request for a six month site restoration extension on 11/19/09 due to inclement weather, and a need for service rig work. The well reached total depth on 2/5/09 and the ID sign and E&S plan was on site. All E&S controls were installed properly and need to be maintained and inspected after every significant rainfall event. The following extension has been approved, and the deadline for the site restoration is 5/5/10. The Department requests that the fill slop be inspected on a regular basis to hinder any accelerated erosion.
