Water in well initially tested up to 4% methane gas, but lowered to 30-40% LEL using Altair 5. Tested water and ambient air in basement of house: 0% LEL for water and all ambient air test locations.

Dean's Water is scheduled to plumb-in water buffalo and deliver fresh water tomorrow AM.

Had to rehead and rewire connections, got camera to work, but had to tape 3 flashlights to camera head to supplement lighting. Unable to record data, as recorder would not work. Vigorous gas bubbling made visibility through water difficult. Here is a summary of what was observed: Abundant water entering well at ~8', below bottom of 16" conductor. Blurred glimpses through bubbling water revealed what looked like casing or tubing along the side of wellbore to 72'. The side of the casing/tubing was missing and tapered to a "V" at 72'. The wellbore deviated from the outside of the casing below 72' and penetrated fresh bedrock.

Yost representative Pat Novak on-site. Yost representative John Mouser arrived later during this inspection.

SDSs are on-site. The Department observed several areas where a small amount of fluids was spilled on containment and/or the timber mats. Mr. Novak confirmed that the well site is on containment; prior to bringing equipment on-site, a 60'x120' piece of containment with berms was installed on-site. Mr. Novak showed a picture to the Department depicting this containment on-site prior to bringing equipment on-site.

Mr. Novak also stated that no grading of the site was performed as part of the site preparation for plugging activities. The Department recommends that E&S BMPs are installed as needed for any earth disturbance activities which may occur or have occurred and that an E&S plan is developed and implemented for any earth disturbance activities w
