Site vegetated with >70% vegetative cover, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

An extension of time for site resotration was approved by the Department on August 11, 2016. A condition of the restoration extension requires the installation of post-construction stormwater management (PCSM) BMPs by November 11, 2016. Chief has requested an extension of time to install the PCSM BMPs.

No violations were cited during this inspection.

2689581 Primary Facility 2018-02-01 Routine/Complete Inspection No Violations Noted One well staged for production phase. One each sand and water separator. No leaks detected from production equipment. No violations noted.
2796345 Primary Facility 2018-10-26 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:45. Steve from Waste Management arrived at the same time. He was dropping off a dumpster near the produced water tanks. He believed that Chief was coming to the pad later that day to clean out the tanks.

There was 1 properly tagged Marcellus well staged for production. There was also 1 GPU, 1 compressor and 2 produced water tanks onsite and in secondary containment. The well cellar contained some water; no bubbling was observed.

The site was >70% vegetative cover and no E&S issues were observed. According to plans onsite PCSM appeared to be constructed and functioning as designed. Some compost filter sock remained onsite (Photos #1, 3).

Department records indicate that the ESCGP does not expire until 11/12/22.
