| | Weather was 40° and sunny what time inspection. There were no chief representatives on location. We had no pressure gauges attached at time of inspection. Allied horizontal wireline was on location to perform bond logs. There were 2 annulus vents plumbed to surface. One was closed in and one (20x13) was open to atmosphere at time of inspection. 0% combustible gas was detected on the annulus vent or in the area immediately surrounding the well. The well tag was removed at time of inspection due to the work being performed however it did not have the wells corresponding API number on it. No violations were noted at time of inspection.

| | Arrived onsite for a routine inspection at 13:15; the weather was ~30°F and calm.
| |||
| There was 1 plugged well and 2 regulatory inactive wells onsite. There were a total of 6 well cellars onsite. One cellar was filled with gravel while the others were filled with water; no bubbling was observed.
| |||
| There was a pipeline run to the pad but no other production equipment was onsite. The pad was well vegetated and no E&S issues were noted.
| |||
| According to Department records the ESCGP for this site expired on 3/20/17 and no NOT has been submitted. However, at the time of this inspection it appeared there were pending well permits.
