Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

As indicated in Chesapeake's response to the Department's June 26, 2015 letter requesting the status of all facilities, a restoration extension request is to be filed for this location. As of the date of this inspection, no request for a restoration extension has been received by the Department of Environmental Protection.

No violations were cited during this inspection.

This location recently changed ownership from Chesapeake to Chief as reflected by pad signs. There two TD'edwells: R&N SW Sul 4H (regulatory inactive status), R&N N Sul 3H (regulatory inactive status). There was also a plugged conductor-only well: the R&N SE Sul 1H, and an additional 3 water-filled cellars.

2792009 Primary Facility 2018-10-18 Routine/Complete Inspection No Violations Noted Weather was 40° and sunny what time inspection. There were no chief representatives on location. We had no pressure gauges attached at time of inspection. Allied horizontal wireline was on location to perform bond logs. There were 2 annulus vents plumbed to surface. One was closed in and one (20x13) was open to atmosphere at time of inspection. 0% combustible gas was detected on the annulus vent or in the area immediately surrounding the well. The well tag was removed at time of inspection due to the work being performed however it did not have the wells corresponding API number on it. No violations were noted at time of inspection.
2803146 Primary Facility 2018-11-14 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 13:15; the weather was ~30°F and calm.

There was 1 plugged well and 2 regulatory inactive wells onsite. There were a total of 6 well cellars onsite. One cellar was filled with gravel while the others were filled with water; no bubbling was observed.

There was a pipeline run to the pad but no other production equipment was onsite. The pad was well vegetated and no E&S issues were noted.

According to Department records the ESCGP for this site expired on 3/20/17 and no NOT has been submitted. However, at the time of this inspection it appeared there were pending well permits.

It is recommended that the well tags be placed on the wells themselves.

No violations were noted at the time of inspection.
