1817896 2009-08-03 No Violations Noted Drilling was completed 2/27/2008. The well site has been restored and is well vegetated. Well is equipped with a pump jack, which is not in operation at the time of the inspection. Brine tank is in sound condition. No signs of erosion and sedimentation on the well site and access road.
1846775 2009-12-01 No Violations Noted According to records, permit was granted 2/15/07. Well was completed on 2/5/09 to a depth of 1142 ft. WRAC report was submitted and approved on 3/21/08. Well tag present but was disconnected from wellhead and needs reconnected. Well head consisted of 8﾿ pipe reduced to 4﾿ to 2﾿ shut off valve. Well head consisted of 8﾿ pipe reduced to 4﾿ to 2﾿ production pipe with 5 shut off valves (in the off position). Production equipment consisted of jack pump connected to well head, propane tank, data logger and two 100 bbl poly brine tanks. No signs of gas leakage around well head. At the time of inspection, no IW or E&S issues at the site.


2155689 2013-04-04 No Violations Noted Well plugged on 11/14/2012. Monument buried. Located in ag field. Shared access road in good condition. No accelerated erosion or sedimentation issues observed in relation to well site or access road.

Site has been regraded and temporarily stabilized with seed/mulch. No vegetative growth observed. As per current regulations, Operator has until 8/14/2013 to achieve full restoration of well site. Department recommends Operator re-seed/mulch site to promote vegetative growth and monitor and maintain site to ensure permanent stabilization and final restoration is achieved.

2195182 2013-07-29 No Violations Noted Well plugged on 11/14/2012. Monument buried. Located in ag field. Shared access road in good condition. No accelerated erosion or sedimentation issues observed in relation to well site or access road.

Access road and pad remain. Pad stabilized with site perimeter permanently stabilized with uniform and adequate vegetative growth.

Department spoke to Operator representative Kent Keppel on 8/5/2013 at 1430. Landowner requested that access road and well pad remain for utilization/storage of farm equipment. Department recommended Operator submit documentation from landowner for verification.

UPDATE: 8/28/2013 Department received above referenced documentation from landowner via Operator representative Kent Keppel.

2204190 2013-09-05 No Violations Noted Bond release inspection. Well plugged on 11/14/2012. Buried monument. Access road stable. No accelerated erosion or sedimentation issues observed in relation to well site or access road.

Site is permanently stabilized and adequately restored to pre-existing conditions. As per landowner request, access road and pad remains (See Attached). No violations noted.

Based on today's inspection, Department is recommending bond be released for said well.
