Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID
1816001 2009-07-21 No Violations Noted spud notif. 7-2-09 - well not drilled/site made - E&S ok
1817843 2009-07-28 No Violations Noted Drilling was completed at the time of the inspection. Below grade pit located onsite had more than the two feet of required freeboard. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. It was noted that the culverts and associated erosion control BMP's had not been compromised. The culvert inlets had been clogged with sediment and would not function properly.
1836020 2009-10-15 No Violations Noted Drilling had been completed and the well appeared to be equipped for production at the time of the inspection. Fill slope had been stabilized with rock, and the filter fabric fence appeared to be holding in place and continuing to function properly. Cut slope had not been stabilized, and the site had not been regraded. In addition the below grade pit was located onsite. The pit had more than the required two feet of lined freeboard, but it appeared surface runoff had been flowing into the pit. The Department recommends that the operator dispose of the cuttings contained in the pit in accodance with the regulations set forth in Chapter 78, regrade the site and implement temporary BMP's to minimize accelerated erosion & sedimentation until final restoration activities can be completed.
