0260568233 2013-04-11 Drill / Deepen / Reopen No Violations Company is swabbing to get the displacement water out of the pipe from the last cement job. They plan on drilling through the cement, then into the producing formation (Berea sand).
0541802100 2016-09-21 Preliminary Restoration No Violations The purpose of my inspection today is a status check on a preliminary restoration. During my inspection the location has been restored, there is no erosion and no barren spots on location. The lease road to the well is gravel. Location around the well and production equipment has thick vegetation growing and the vegetation has been kept mowed. The well is equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, 2.375 inch diameter tubing, and a tubing valve in the opened position . The well is producing. The production equipment for this well consists of one 100 barrel steel tank, one chart meter with chart dated 9/1/16, and one vertical separator. All production equipment is within an earthen dike and identification is posted on the 100 barrel tank. No violations present during my inspection, location has met 1509.072 A requirements.
1353071196 2012-11-14 Drill / Deepen / Reopen No Violations I was out to inspect rig this day as has started drilling. Pits look good, permit no cable tool rig, crew working on rig when I arrived. They had only drilled 35 of surface hole and could be week to several weeks before ready to cement surface, due to Holiday, hunting season. Surface casing on location - have verbal permission from S. Opritza to use it and weld it together - permitted before new rules. Looks like good, unpitted casing.
1358246640 2013-01-12 Drill / Deepen / Reopen No Violations Bob Lane called to say the cable tool rig had made it thru the red beds and will run 8 to 875 on Saturday - 1/12/13. They will not cement it but will pull out before cementing the 4.5 casing. They will also run a string of 7 which will also be pulled out. This well falls under old construction rules, per Steve Opritza.

1903281714 2013-04-02 Drill / Deepen / Reopen No Violations Witnessed the cementing of the production string. Good circulation was established. After cement was pumped and the wiper plug landed, they pressured up on the pipe, and locked the valves down. Pressure held for 15 minutes.
