0043074452 2015-11-17 Plug / Plug Back No Violations On location to witness plug back operations for this well. The 4.5 casing is parted at 2426 and 1 joint was pulled, leaving the casing at 2381. Formation Cementing set a 200 rip isolation plug from 2381-2181 using 60 sks class A cement with 2% CaCl, 30 lbs cottonseed hulls as LCM, and 34.6 bbls of saltwater displacement. Good circulation was established with saltwater before pumping the plug. The fluid in the well equalized after shutdown, indicating the plug balanced. Jet Wireline Services will be on location to tag cement and run a cement bond log from the bottom of the 8.625 surface casing to surface.

Last week, Appalachian Well Surveys ran a bond log of the top of the Big Lime formation and perforated 10 holes in the 4.5 casing over 10 from 4617-4627 in preparation for the Big Lime plug. Heinrich Enterprises rig crew set 1.5 tubing at 4648. Formation Cementing set a 200 Big Lime plug from 4648-4448 using 55 sks 1% gel cement and 11.2 bbls of freshwater displacement. Tubing will be pulled OOH and a 150 rip isolation plug will be set where the 4.5 casing is parted.
0465408693 2015-10-29 Plug / Plug Back No Violations On location to witness plug back operations for this well. Heinrich Enterprises set 1.5 tubing at 6302, below the Clinton Sandstone perforations. Formation Cementing pumped a 500 bottom plug using 40 sks class A cement with 2% CaCl, 30 lbs cottonseed hulls as LCM, and 14.6 bbls of freshwater displacement. The well went on a slight vaccum after the plug was pumped then went static, indicating the perforations were sealed off. Tubing will be pulled OOH in preparation to tag the bottom plug, log the Big Lime section, and perforate the 4.5 casing 100 into the Big Lime.
1049617570 2015-10-22 Plug / Plug Back No Violations On location to witness plug-back activities for this well. Heinrich Enterprises is using their own service rig for the plug back. 1.5 tubing was set to 6308. Formation Cementing began pumping on the tubing to make sure it was clear before pumping a 500 bottom plug. The pump truck pumped 6 bbls into the tubing and pressured up to 1100 psi, indicating a blockage in the tubing. Several attempts were made by Formation to clear the blockage, with no success. Heinrich Enterprises will pull the tubing OOH and examine it to identify where the blockage occurred. No plugs will be set today.

1681948301 2017-07-20 Production Wells No Violations The inspection this day revealed that this well is equipped with a plunger lift system, a 4 1/2 inch diameter production casing, and 2 inch diameter production tubing. The valves on the wellhead are open at the time of inspection. The tank battery for this well is equipped with a 210 steel tank with identification on it that is beginning to wear away but is still legible. There are no issues to report at this time and the well is in production at the time of inspection.
1911641878 2017-01-04 Plug / Plug Back No Violations Inspection of this well found a pit lined with plastic and 3-210 barrel tanks. The tubing was not connected to anything and closed at the valve. This well was a plug back from the Clinton to the Berea with an exemption and special permit conditions for perforating the surface string. This well was plugged back and the Berea was perforated in November of 2015. The pit has stood open and the pit liner has been weathering and its integrity has been compromised since then. The pit shall be closed properly per Ohio Revised Code and, as noted on permit, Inspector shall be notified. Further thought should be given as to whether to finish plugging this well or to try to produce the Berea formation that was perforated. The permit expires in August of this calendar year. With a decent road that has gravel weather should not affect the closing of the pit and work shall be completed by the 23rd of January 2017.
