0581300034 2017-09-27 Final Restoration No Violations The Boyas Excavating # 2 shared the tank battery with the Boyas Excavating # 1 ( 34-153-2-2421 ) and was found to be compliant with the Final Restoration criteria. The area where the tank battery existed was graded to the surrounding native contours. Vegetative growth was well established and stabilizing the soil. All equipment and material was removed from location. The location of the wellhead was buried under 20 of construction debris. A Landowner waiver ( Form 5) was executed by Cuyahoga Street Development LLC to allow the flowline to remain buried in place. The flowline was purged with fresh water ( per DOGRMs John Fleming ). All criteria for Final Restoration was met.
2082693195 2017-09-27 Final Restoration No Violations I was on location with Inspector Bob Roush to perform the Final Restoration of the above well location and tank battery. All equipment has been removed. There is no production equipment at the tank battery location. Vegetation has been established The well head site is being use to receive dirt from the Akron sewer project.
