0603699274 2014-09-24 Production Wells No Violations I was on location to follow up on complaint #4802 regarding black in the water well and tremors at a home located at 1517 Mattie Street SE, Canton, Ohio. The tank battery contains a 210bbl production tank, brine vault, vertical separator and an ID sign. The well is a plunger lift well with no gauges to measure pressure. The 8.625 diameter casing is buried and has a 2 diameter riser pipe to surface. There is no gas flow or pressure on the 2 riser from the 8.625 wellhead. The well and the tank battery are in good condition and appear to have no mechanical issues.
1304409014 2018-08-09 Production Wells No Violations I did a status check inspection on this well. The well is equipped with a casing plunger-lift system. The tank battery consists of a steel production tank, an inground concrete brine vault, a horizontal separator and a meter. The well is legibly identified. No compliance issues are noted at this time.
1320635260 2011-11-04 Production Wells No Violations I received a call on 11-4-11 from Deb Moore of the Stark County Health Department (330-493-9904, extension 260) regarding a water well investigation she is conducting for Glen Sheats and Helen Stevens who live at 1701 Mattie St., Canton, OH 44707. The water well tested negative for bacteria and negative for VOCs. The Health Department does not test for methane and she would like ODNR to test for methane. The home owners told her that they saw gas bubbling around the casing of the oil and gas well over a year ago. Not long after that they started to see effervesence in their well water and became concerned. I met with Tim McConaha (Enervest) and the property owners and inspected the oil and gas well. There was no evidence of gas leaking at the wellhead. I requested that Enervest dig up and expose the buried 8.625 wellhead so we can check for any gas leaks. This will be done on Tuesday 11-8-11. The water well is located approximately 350 north and upgradient from the oil and gas well. I went into the basement of the house with the home owners to do a visual inspection of the their water. The water is highly effervesent, turns clear in less than 1 minute when put into a glass, and has no petroleum odor. There was no evidence of oil residue in the toilet tank or in the sink. I will investigate the buried oil and gas wellhead for any leaks and I will ask Kyle Tharp to contact Deb Moore about sampling the water well for methane.
1320885840 2011-11-08 Production Wells No Violations Hawkins Well Service excavated around the 8.625 wellhead. There was no gas leaking from the wellhead, but there was some oil staining in the excavated soil. This is normal for wells that have been in production for many years. Hawkins Well Service installed a 2 dia riser pipe from the 8-5/8 dia wellhead to 2 above ground level for future monitoring of gas. The excavation was backfilled and leveled. Glen Sheats, Helen Stevens, Charlie Youngen (Enervest), and I observed the excavation process. I walked up gradient from the oil and gas well and took a GPS (40.75986, 81.35888) on the water well.
1608059990 2015-03-10 Production Wells No Violations I was on location to follow up to complaint #4957 regarding natural gas venting from a domestic water well. The lease road is stable and does not show signs of erosion. The tank battery is contained within a dike and has proper I.D. signage. The tank battery contains a 210bbl production tank, vertical separator and concrete brine vault. The well is a concoyle well with a riser tube from the 8.625 diameter surface casing. There is no pressure or venting from the annulus. The well is in good mechanical condition and there are no violations at this time.
