Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1139945926 2006-02-01 Production Wells Violations Noted The leak has been exposed and repaired. The violation is corrected.
1242142337 2006-01-12 Production Wells Violations Noted The line leak closest to the pond has been exposed, clamped and burried. The second leak still needs to be corrected.
1514209028 2006-01-11 Production Wells Violations Noted On January 10 and 18:00, I received a call from Bart Raye with OEPA reporting an oil spill near 9624 US 224, east of the Deerfield circle. EPA was contacted by Larry Carver (330-889-3944) at that address reporting oil in the corn field. He was concerned that the oil might reach his pond. I contacted Mr. Carver and Mitch Fitch with CGAS who in turn met with his pumper to shut the two Murphy wells in and place sorbent pads and booms between the spill and the pond. Pipeline repair and spill remediation would resume in the morning. During this inspection the next morning, there appeared to be two leak locations approximately 500 feet apart. The leak further south probably is associated wth the sales line and and brought less than one gallon of oil to the surface. The second leak located closer to the pond may be from the flowline and appears to have resulted in the loss of 20 to 30 gallons of oil. At no time did either line leak impact the pond. CGAS will expose and clamp the lines and evaluate the system for possible replacement.
1919060021 2016-12-22 Production Wells No Violations I conducted a status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped and operating with the aid of a natural gas-powered pumping unit. The wellhead and unit were fenced and the required ID is posted on the fencing. Gauges at the wellhead indicate a pressure of 65psi on the 4.5 production casing and 65psi on the tubing. A gauge on the annulus between the 4.5 casing and 8.625 surface casing shows a pressure of 75psi and is supplying gas to the pumping unit motor. This well produces to a diked tank battery to the north and is shared by the Murphy-Waltz Unit #1 (PN# 22133). The battery contains one (1) 210 barrel tank, one (1) 100 barrel tank, two (2) horizontal separators, and a shared brine vault. This well produces through a dedicated horizontal separator to the 100 barrel tank. The proper ID is posted on the tank. I found no violations at this time.
