0299468562 2015-10-07 Production Wells No Violations Upon my follow up inspection I found that all contaminated soil has been removed from around the well and all equipment was repaired and functioning. The well jack was pumping at the time of arrival to the well and I found there to be no other violations at this time.
0861707895 2018-09-10 Production Wells No Violations I was on location today to do a field inspection on this well. The well does appear to be in production. There is a pumping unit on this well with rods and 1.5 inch diameter tubing in it. The production valves are open from the wellhead to the tank pad. The identification for the well is located at the well and the tank pad. There are no violations at this time.

1261537040 2015-09-21 Production Wells Violations Noted On my inspection today I was accompanied by Inspector Smith. During the time of my field inspection we found the stuffing box was leaking causing contamination to the soil. All contaminated soil must be removed and the leak must be fixed.
1412090659 2014-09-30 Annular Disposal No Violations An inspection of this well was requested by Central Office to verify Annular Disposal (AD) connection. AD is not connected to the wellhead at this time.
1439312241 2017-01-19 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter wellhead, 1 1/2 inch diameter production tubing, and 2 inch diameter steel production lines. The valves on the well head and production tubing were open at the time of inspection. Near the wellhead there is a meter box with a current chart on it and, the chart was recording at the time of inspection. There is a small separator buried in the ground near the meter box with its valve open. The tanks for this well is at the entrance of the well. The tank battery consist of four (4) tanks two 210 barrel steel tanks, one 100 barrel steel tank, and a 50 barrel plastic tank. Two tanks, a 210 barrel and the 100 barrel tank, are labeled for the Clarence Hess 4-31 leaving me to believe they are used strictly for this well. Each tank has individual identification on it for the well that leads to it. There are no issues to report and the well is in production at the time of inspection.
