1170784101 2007-02-05 Called by Andrew King to report an oil spill in a wetland west of Sharon Community Park in Sharon Center. A two inch flow line split outside the dike and approximately two barrels of oil spilled in marsh area. King notified the OEPA and contractors were on site within two hours. I arrived and assessed the contamination. Spill was contained for the evening. 2-6-2007 began removal of trees in oil. Peatsorb was applied to the oil on the water. Trees and contaminated soil will be removed from site after oil is absorbed. Tank battery will be removed and built at a higher elevation. Kurt Kollar of the OEPA arrived to instruct King on the water cleanup issue. ODNR and OEPA will work together to bring this in compliance. Cloudy, -5 degrees. 9-10-04

1171988948 2007-02-20 Well remains shut down due to oil spill clean up. Removing 210 barrel storage tank and built siphon dams to collect oil. 35 degrees12 snow.
1176403000 2007-04-12 Oil contaminated soil has been removed and hauled to Petro Enviromental in Lodi. Seven tons removed. Kurt Kollert of OEPA advised me all water contamination is in compliance. King Drilling has set a new tank battery and dike. Area needs final graded and seeded. Due to weather conditions I gave another extension for final grade.
1179761777 2007-05-21 Case# 07-002. OEPA has released their responsibility on complaint. Tank battery has been replaced and ground is dry. Met with Sharon Township trustee on site to discuss final grading of site. Production oil line is still exposed and needs protected before grading over line. Andrew King and township will notify me when area is reclaimed. Sun, 60 degrees.
1186679610 2007-08-09 NOV in compliance. All oil contamination removed new tank battery instaled and are graded and vegetation growing. Rain, 88 degrees.
1361476132 2013-02-21 I inspected this well and found one pump jack with a natural gas powered motor on the wellhead. The pressure on the tubing was 12 psi. and the pressure on the casing was 20 psi. There was a small steel tank used for injecting well treating chemicals plumbed into the tubing. No leaks were observed on the wellhead or piping around the well. The area around the well was nicely maintained and the wellhead and pump jack unit were enclosed with a three sided tall wooden privacy fence. The wellhead, pump jack and wooden fence were surrounded by tall pine trees concealing the presence of the well. The tank battery contained one 100 bbl. steel oil storage tank and no other production equipment. The proper identification sign was posted on the tank and no leaks were observed on the tank or pipimg within the tank battery. The dike was in good shape and the area within and around the dike was nicely maintained. The tank battery was enclosed on three sides with a tall wooden privacy fence. It was determined that this tank battery also handles production for the Sullivan #1, #2 and #3 wells.
611623797 2007-03-12 Met with Andrew King on site. Due to melting snow and rain the oil in wetland is contained and crews are still mopping up oil in water under supervision of OEPA. Discussed construction of new tank battery location at NE corner of park, parking lot. 210 barrel tank removed and new 100 barrel brought in. Cloudy, 45 degrees.
