0964062791 2004-03-17 Production Wells No Violations The Tank battery consists of 1-100 bbl storage tank and a dike.
1094779760 2010-07-21 Production Wells No Violations I performed a random field inspection. I GPSd both the tank battery and the wellhead. I continued to walk around both the wellhead and the tank battery. This inspection satisfies 1501:9-9-05. There were no violations at this time.
1584670554 2018-12-18 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0391. Inspector John Fleming and lease operator Ron Stiles were present during the inspection. RBDMS indicated reported production was current. Access remained drivable and stable. The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. The tank battery consisted of one (1) horizontal separator and one (1) production tank. The separator remained in good repair and contained a current 7 day chart. A stable earthen dike that was free of fluid was present around the tank. The tank remained in good repair. Valves were closed and plugged. Identification present on the tank contained the wrong permit number. Mr. Stiles acknowledged the issue and stated he would replace the identification sign. Mr. Fleming will conduct a follow-up inspection to document the corrected identification sign.
