The permanent fence around the school ground site provided sufficient security for the public. The blowout preventor was properly installed. The pit liner was found to have slumped. However, there was no evidence that it was compromised and the driller agreed to replace the liner as a precaution. Some liquid was found, on the parking lot, around and under one of the tanks. The rig crew was instructed to clean up with some pads. There were no violations found at the time of inspection.

1404433920 2017-07-31 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice. At the time of the previous inspection, I had noted the following: the unit ID signage was not complete and the load-line valves were not secure with locks. At the time of this inspection, the noted deficiencies remain. I contacted John Magnussen with David Oil who indicated that the issues are being handled as they are able. I inspected the wellhead and surrounding area and found no signs of problems otherwise.
1406657068 2014-07-29 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 08/05/2009. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and pump jack system with a gasoline engine-fired motor. The wellhead and battery are located within the paved asphalt area of the school maintenance facility. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located 300 feet to the west, in a separate board-on-board enclosure. The battery is comprised of a total of 1 steel 210-barrel production tank, 1 140-barrel steel brine tank, and horizontal separator in a 3rd, separate, locked enclosure. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged and locked. The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is process safety equipment present (low-pressure relief valve, carbon filter, and spark arrestor). There is ID signage at the battery which is legible and in good condition. The area surrounding the wellhead and battery is comprised of asphalt pavement and is flat-lying. I did not observe any signs of drainage or erosional deficits associated with the unit.

The well and battery are located within the maintenance facility and bus parking area for the City of Mentor Schools. The well, battery, and mechanical separator are each enclosed in separate board-on-board enclosures. At the time of this inspection, the gates are locked and secure. The signage is present and emergency contact numbers are current. Fittings and connections at the battery are secure with no signs of leakage or problems. Fittings and connections at the wellhead are secure. There is some minor paraffin staining around the wellhead but there is no free oil or leakage. The secondary containment at the battery is intact, dry, and free from signs of leakage,

The spacing is in accordance with the rule, and the valves and mechanicals are in good repair. I did not observe any signs of leaking product or gas vapors. The wellhead is fitted with a mechanical plunger lift and is located 75 feet souoth of the TB. The mechanical separator is located within a separate enclosure. The tank battery dike is adequate. The gate to the TB is unloked. The ID on the production tank does not match the fence. I called Tim Reilly and asked him to put locks on gates and remove the improper sign.

2034531333 2016-02-18 UrbanProduction Wells Violations Noted I inspected this producing well as part of routine inspection activity. At the time of my inspection, I observed the following elements: a wellhead with pump jack system and tank battery consisting of one 210-barrel steel and one 100-barrel steel stooge tank, with mechanical separator. The battery and wellhead were in in separate board-on-board enclosures. Fittings and connections are free from leakage. Access is via the shared asphalt-paved parking and drive area for the service building. The surface casing annulus is not gauged as required by permit condition. Gates was secure and locked. The load-line valves are not locked and should be secured with locks. The ID signage is partially missing. The owner must remedy the following by 04/01/2016: secure the load-line valves with locks. Repair the missing signage. Properly affix a gauge at the surface casing annulus. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
