| I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: the load-lines were not secured with locks and the tank spacing within the battery was not compliant . In addition, the identifying signage had remained unchanged since the previous change of ownership and had not been updated to reflect new ownership. During a subsequent inspection, I had observed that the production tank had become corroded with several perforating holes rendering the tank not capable of preventing a release. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.

| I inspected this producing well as part of routine inspection activity. At the time of my inspection, I observed the following elements: a wellhead with mechanical plunger-lift system in a chain-link enclosure which is locked. The enclosure is located in an otherwise undeveloped parcel in an area of mostly light industrial development.  The tank battery is comprised of one 100-barrel and one 50-barrel tank, and mechanical separator in a chain-link enclosure.  Fittings and connections at the battery were secure. Tank lids were closed and secure.  Enclosure gates were not locked. Load line valves were not secure with locks. The tank battery is located less than the required minimum distance of 50 feet from the well head. The owner must remedy the following by  04/04/2016:  Lock the enclosure gates and load line valves.  Correct the tank spacing at the next well work-over activity. | I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves and violation of the tank spacing requirements.  At the time of this inspection, I observed the following: The load-line valves were not secured with locks and the production tank remained less than 10 feet from the mechanical separator unit. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I also noted that the production tank had a perforation resulting gin complete loss of integrity. I issued a separate compliance notice for the tank. I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible.  I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.

| I inspected this location pursuant to a notice of a possible fire and subsequent confirmation with the Lake County Emergency Management Agency at 8:45 PM. Upon my arrival, crews from the City of Mentor Fire Department (MFD) and from Great Plains Exploration were on site. According to MFD site commander, it is believed that lightning struck a portion of the tank battery resulting in a fire associated with the 100-barrel brine tank, which is comprised of fiber-reinforced plastic (FRP). | I inspected this location pursuant to a notice of a possible fire and subsequent confirmation with the Lake County Emergency Management Agency at 8:45 PM. Upon my arrival, crews from the City of Mentor Fire Department (MFD) and from Great Plains Exploration were on site. According to MFD site commander, it is believed that lightning struck a portion of the tank battery resulting in a fire associated with the 100-barrel brine tank, which is comprised of fiber-reinforced plastic (FRP). In addition, the identifying signage had remained unchanged since the previous change of ownership and had not been updated to reflect new ownership. During a subsequent inspection, I had observed that the production tank had become corroded with several perforating holes rendering the tank not capable of preventing a release. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.

At the time of my inspection, I observed the following elements: a wellhead with mechanical plunger-lift system in a chain-link enclosure which is locked. The enclosure is located in an otherwise undeveloped parcel in an area of mostly light industrial development. The tank battery is comprised of one 100-barrel and one 50-barrel tank, and mechanical separator in a chain-link enclosure. Fittings and connections at the battery were secure. Tank lids were closed and secure. Enclosure gates were not locked. Load line valves were not secure with locks. The tank battery is located less than the required minimum distance of 50 feet from the well head. The owner must remedy the following by 04/04/2016: Lock the enclosure gates and load line valves. At the time of this inspection, I observed the following: The load-line valves were not secured with locks and the production tank remained less than 10 feet from the mechanical separator unit. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I also noted that the production tank had a perforation resulting gin complete loss of integrity. I issued a separate compliance notice for the tank. I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.
