0163215738 2018-03-13 Preliminary Restoration No Violations On the day of my inspection I found a Universal frac crew setting up the pad for frac job. Water tanks and containment are onsite, no frac pump. 7-15 frac stacks are on all wells. 2H, 5H, and 7H are the first wells planned for stimulation. Outslopes of pad are vegetated and stable. Site meets requirements of ORC 1509.072A for preliminary restoration.
0345495491 2017-08-30 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Falcon Drilling rig 26 drilled a 24 inch diameter hole to a depth of 185 feet and set 170 feet of 18.625 inch, 87.5 pound per foot casing. O-Tex pumped 30 barrels of gel then 425 sacks of class A cement, 1.2 yield, 15.6 pound per gallon, 2 percent Calcium Chloride. Two voids were identified from 103 feet to 109 feet and from 130 feet to 135 feet, two baskets were set at 96 feet and 98 feet to avoid the upper void (conductor to 100 feet). O-Tex displaced with 49 barrels of fresh water to land the plug. Cement did not circulate to surface. Falcon ran approximately 88 feet of one inch tremie pipe down the backside of the 18.625 inch casing. O-Tex pumped 9 barrels of class A, 1.2 yield, 15.6 pound per gallon cement to set the basket. They waited one hour then attempted to pump more cement but the truck broke down. Another truck was brought to the site and they topped out with 70 barrels (369 sacks total to top out). On the third top out cement returned to surface and remained static.
1298204389 2018-06-12 Preliminary Restoration No Violations On the day of my inspection I witnessed that the pad has been brought online. Pad has 5 properly labeled wellheads. Production equipment includes two 500 barrel steel tanks, five GPUs, two line heaters (for the 5H and 7H wells), and seven sand seperators. These wells do not have tubing in them and are produced through the 5.5 inch diameter production casing. Set up appears to be a frac stack with well head on top of it. Flowback operations onsite have a target goal of 31.9 mmcf/day for this well but the company man Chad Patin does not think it will do over 27 mmcf/day. Wells with target goals over 20 mmcf/day have an additional line heater to supplement the GPU. Surface annular pressure is 275 psi, intermediate annular pressure is 726, and production pressure is 5277 psi. Flowback is finished on this well. Pad is stable and vegetated. Site meets requirements of 1509.072A for preliminary restoration.
1801428147 2017-08-31 Drill / Deepen / Reopen No Violations Today I am on site to witness the second hydrostatic test on the conductor cement job. Falcon Drilling rig 26 drilled to 192 feet, 7 feet below the last total depth, and loaded the hole with water. I witnessed the water level remain static in the pipe for 30 minutes.
