0211335402 2018-12-04 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. Wireline was coming out of this well after setting stage 26/67 plug at 14743. Bottom perforation at 14734 and top perforation at 14613. Surface annular pressure is 0 and intermediate annular pressure is 200.
0212027164 2018-09-05 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Patterson 795 drilled a 17.5 inch diameter open hole to a depth of 1430 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming Casing ran 1433 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot. Shoe track is 43 feet long. Schlumberger was onsite with 1275 sacks of class A surfaceset cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 20% excess ran in open hole. Cement displaced with 214 barrels of fresh water. 25 barrels of gel and 5 barrels of water pumped ahead of cement. 49 barrels of cement returned to surface, and floats held with 1 barrel back.
0227485115 2015-05-07 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the conductor. I was informed by Precision Rathole that they had drilled a 36 inch diameter hole to a depth of 100 feet. Precision set 100 feet of 30 inch diameter, 118lbs/ft conductor at 95 feet in depth. After attempting to ream the hole clean they were still unable to get the last five feet on bottom due to drilling through mine spoil. I field titrated mix/displacement water with a Quantab and found the level to be 32ppm. Allied Oil & Gas Services pumped 600sk of Class A cement, 15.6ppg, 1.2 yield, 2% CaCl2, and 0.25lbs/sk of flake. Allied displaced cement with 76bbl of water. 15bbl of cement returned to surface and stayed at surface while I was on site.
0594095136 2018-09-18 Drill / Deepen / Reopen No Violations On the day of my inspection I was accomapanied by Inspector Ken Fry. We identified ourselves to company man Larry Byrd. Patterson 795 reached total depth with an 8.5 inch diameter bit at 18554 feet. Wyoming casing ran 18556 feet of 5.5 inch diameter, 23#, p-110 casing. This is a SIMOPs pad. Annular pressures on the temporarily abandoned well are zero psi. Company man did not have anticollison reports and directional driller was not on location during the inspection. Rig is waiting on bulk cement from Schlumberger.
0788017984 2019-05-08 Preliminary Restoration No Violations On the day of my inspection I visiting pad at the request of Adam Schroeder on behalf of Mr. Humpreys. Mr. Humpreys was concerned with no production on the 9H well which was due to shutting it in to drill the 1H, 3H, 5H, and 7H wells. Email was sent to Adam Schroeder informing him the well in back in production. This well is currently producing off of natural reservoir pressure. Production equipment includes five GPUs, five sand seperators, two line heaters, one combustor, three compressors, two vapor recovery towers, and eight 500 barrel steel tanks, within their own containment. The 13.375x9.625 annular pressure was 0 psi and the 9.625x5.5 annular pressure is 2000 psi. Burst pressure on the 13.375 casing is 3090 psi. Spoke with lease operator onsite and he said he would get the annular pressure blown down. Pad is stable and vegetated. Site meets requirements of 1509.072 (A) for preliminary restoration.

1381148779 2019-01-29 Completion Testing No Violations On the day of my inspection I witnessed CWS rig 7009 onsite. This well has not had frac plugs drilled out yet. They will run 2.875 inch diameter tubing after drilling out. Surface annular pressure is 0 and intermediate annular pressure was 0.
1515005786 2018-03-15 Drill / Deepen / Reopen No Violations On the day of my inspection I found this pad to contain 1 properly labeled producing well and four conductors/cellars that are not labeled. Production equipment includes one GPU, a vapor recovery tower, combustor, compressor, and four 500 barrel steel tanks. Steel tanks are in metal containment and there is a berm around pad. There is no drilling equipment onsite. Outslopes of pad are stable and vegetated. Pad is fenced and locked. Per ORC 1509.072A this permit is not eligible for a preliminary restoration until it is drilled to total depth.

1651489383 2019-01-03 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew performing a final pressure test on kill plugs in the 1H well. This well had two kill plugs set in it on 12/14/18 at 8125 and 7975. Plugs passed a negitive and a positive pressure test. Halliburton will be moving off of location starting today. Surface and intermediate annular pressures are 0.
1786491953 2018-11-27 Completion Testing No Violations On the day of my inspection I witnessed Halliburton frac crew on location. Identified myself to company man Justin. They just finished stimulating stage 2/67. Plug was set at 18343, with bottom perforation at 18334 and top perforation at 18213. Universal pumped 5656 barrels of clean water and 1000 gallons of 7.5% HCl. 60200 lbs of 100 mesh and 195865 lbs of 30/70 mesh for a total of 265065 lbs of sand. Break down pressure was 7836 at 15 barrels per minute (BPM). Average rate was 53 bpm with a max rate of 60 bpm. Average pressure was 7836, with a max pressure of 11590. Initial shut in pressure (ISIP) was 4818. Intermediate annular pressure was 200.
1905428928 2018-12-20 Completion Testing No Violations On the day of my inspection I was accompanied by Geologist Tammy Richards. A halliburton frac crew is on location. Equipment is down for maintenance and no frac is ongoing. Surface annular pressure is 0 and intermediate annular is 0.
