0409092704 2016-01-11 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a production tree. Production equipment for the well included a sand trap, a gas production unit and a line heater. Production equipment that was shared with permitted wells 34067212280000, 34067212290000, 34067212310000, 34067212320000, 34067212330000, 34067214310000, and 34067214320000 included a vapor recovery system, 5 gas compressors, a gas combustor and 12-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1101037777 2016-02-18 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 5 compressors

There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1418658000 2014-12-15 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Express Drilling drilled a 26 inch diameter hole to 120ft and set 120 ft. of 94 lb. casing. Express Energy Service cemented with 230 sacks of class A cement at 15.6 lb. and a yield of 1.18 and had 19 bbl. Of good cement returns back to surface.
1425577266 2015-01-05 Drill / Deepen / Reopen No Violations I was on location with Regional Supervisor Tom Hill to inspect drilling operations of Nomac #73. We met with the company men and they are currently drilling the 8.75 vertical section at a depth of 2731. The mud up point will be approximately 7300. We inspected the backyard, the shale shakers and the blow-out-preventer (BOP) stack in the cellar. The location is in good condition and all drilling equipment is on containment. No violations.
1636481518 2015-01-16 Casing String Witnessed No Violations I returned to location at 1:30pm to witness the cementing of the 5.5 production casing. O-Tex Pumping rigged up to the Blackhawk cement head and pressure tested the lines at 3:25pm. They conditioned the hole with 50 barrels of Barite spacer and cemented the casing with 274 barrels of lead cement and 574 barrels of tail cement. They dropped the plug and displaced the plug to the collar with 387 barrels of water. The plug landed at 2450psi and the pressure was increased and held at 3050psi for five (5) minutes. The pressure was released at 6:50pm and the floats held with 4.5 barrels flowing back to the truck. No violations. See cement ticket for more details on this job.
2104597952 2015-01-16 Drill / Deepen / Reopen No Violations I was on location to check the status of Nomac Rig #73. Wyoming Casing was currently rigged up and running 5.5 production casing. They are washing down the last 10 joints of casing. O-Tex and Blackhawk are on location and will rig up when the casing is down and Nomac finishes circulating. Primary cementing operations should commence in several hours. No violations.
