| Southeast geologist Jake Glascock requested that an inspection be conducted as part of a water well investigation. Inspector Moreland and I found the well has been completed but not placed in operation. The production tree has all the valves that operate the well shut in and locked. The tin horn has filled to ground level with rock, there are no signs of leaks spills or contamination and pollution around the well head. There is no production facility in place at this time.

| On the day of my inspection I witnessed the lease road and well location to be in good repair. Upon my arrival on location I discovered that all production equipment has now been put into place and tied together. The permitted well bore is set up to be utilizing natural reservoir for means of production. The wells were not tied into a gas sales line at the time of my inspection. | On the day of my inspection I witnessed Chevron is in the process of putting the well into production. While on site I spoke to an employee working on the equipment who informed me to his belief Chevron will have the well put into production by the middle of January 2017. He stated that they should finish pluming all the equipment in by the end of November but the gas sales line will not be ran to the well until January. Over the duration of my inspection I witnessed no signs of pollution or contamination.

| On the day of my inspection I found this pad with two properly labeled plunger lift wells. Production equipment includes eight 400 barrel steel production tanks, 2 gas production units, one compressor, one line heater, and a vapor reocvery system. Surface casing has 100 psi on it while intermediate has 900 psi.  Zero production shown in 2016, nothing submitted in 2017. Truck is being loaded with condensate while onsite. 0657083570 2015-11-02 Production Wells No Violations Southeast geologist Jake Glascock requested that an inspection be conducted as part of a water well investigation. Inspector Moreland and I found the well has been completed but not placed in operation. The production tree has all the valves that operate the well shut in and locked. The tin horn has filled to ground level with rock, there are no signs of leaks spills or contamination and pollution around the well head. There is no production facility in place at this time.
0716811875 2016-12-13 Preliminary Restoration No Violations On the day of my inspection I witnessed the lease road and well location to be in good repair. Upon my arrival on location I discovered that all production equipment has now been put into place and tied together. The permitted well bore is set up to be utilizing natural reservoir for means of production. The wells were not tied into a gas sales line at the time of my inspection. The production equipment for the well bore consists of the following:
8- 500 barrel production tanks 

2- Vapor recovery unit compressors 1- Condensate flash unit 2- Gas production units equipped with an internal heaters and sand separators 1- Gas sales line heater 1- Sales line compressor 1- Low pressure separator 1- Combustion unit 2- Permitted well bores set to be utilizing natural gas pressure for means of production The site passed preliminary restoration in accordance with ORC 1509.072 A.

0761748468 2014-01-06 Drill / Deepen / Reopen No Violations I arrived on location and found rig drilling, I walked location and found no violations this date.
1344720865 2016-07-08 Production Wells No Violations This well does not have any equipment on site. As of this inspection there is a frack valve set up on the wellhead with nothing connected to the well . There are no tanks or any other equipment that could be used to operate the well.
1422314028 2016-10-28 Surface Facility Construction No Violations On the day of my inspection I witnessed Chevron is in the process of putting the well into production. While on site I spoke to an employee working on the equipment who informed me to his belief Chevron will have the well put into production by the middle of January 2017. He stated that they should finish pluming all the equipment in by the end of November but the gas sales line will not be ran to the well until January. Over the duration of my inspection I witnessed no signs of pollution or contamination.
1523811878 2017-10-31 Production Wells No Violations On the day of my inspection I found this pad with two properly labeled plunger lift wells. Production equipment includes eight 400 barrel steel production tanks, 2 gas production units, one compressor, one line heater, and a vapor reocvery system. Surface casing has 100 psi on it while intermediate has 900 psi. Zero production shown in 2016, nothing submitted in 2017. Truck is being loaded with condensate while onsite. Spoke with James Steel, well tender.
