Today there is a crane on site that is setting the tree on each well. Nothing further is being done on site today and there are no rigs or equipment other than the crane.

| I am on site today to run the E.M 31 conductivity meter with Tom Tomastic across the location. When I arrived I found that the containment dike which was put in place to catch chloride contaminated fluid was full. I contacted Vince Cox for Gulfport and advised him that the system is full of fluid and needs to be pumped off. He arrived and said that the trucks would be on site in the next hour. We conducted a grid pattern and completed the running of the E.M. machine. Tom is to forward the results to Deputy Chief Bob Worstall and Chief Simmers.

Yost Drilling Rig # 7 set 354 ft of casing and Stingray Cementing Service c/w 570 sks of Class A  cement at 15.6 lb . The TD of the 24" inch diameter well bore was 365 ft. During the installation of the casing they hit an obstruction and then decided to shorten the casing to the 354 ft. Had 40 bbls of good cement returns to surface.

| I had a call from Anthony Miller of Gulfport Energy. Antony was advising me of a on going contamination problem occurring on the B.K.Stephens pad. He  informed me that he had this same contamination occurring on as many as 4 drilling pads of Gulfport Energy. I was later contacted by Deputy Chief Bob Worstall and advised to conduct an inspection of this pad and to work with Kyle Baldwin collecting samples and photographs. Kyle is a sampling expert for the State of Ohio Oil and Gas Division. We collected water samples and photographs of the possible contamination along with quantab titration samples for chlorides at 40.18508 -081.16769 and at 40.18521 -081,16823  . The field results from the quantab showed that the chloride range for the fluid sampled were in excess of 6,000 PPM.  Kyle Baldwin collected the samples and transported them to our Cambridge Lab for testing. The source of this contamination had not been determined and no corrective action has been requested until the results from the samples are completed.

| Met with Deputy Chief Bob Worstall and Supervisor Tom Hill on site. I am meeting today to show Bob the amount of contamination coming from under the containment liner. It is running down along the rock pad and over the hill. There was not  to be any action taken to remediate this site until after our inspection. We found that the contaminated  soil below the pad has been replaced with clean fill, seeded and mulched. There is to be a ditch cut along the bottom of the rock pad to allow fluid running from under the containment liner to run into this sump area. The sump shall be lined so that the liner catches fluid running from under the pad, but does not run under the liner. All fluid collected in the sump shall be disposed of at a proper U.I.C. facility or until the chloride range is below 250 P.P. M. The amount of fluid captured in the sump shall be recorded . After the well is drilled the Sidewinder rig shall move off and no further work conducted on site until the clean up has been completed. The water wells up to 1500 ft away shall be sampled again before June 7th.

At this point we are going to make containment of the contamination and wait until the rig is done drilling. | I received a call from Hal Miller of Gulfport to inform me of a spill on the B.K.Stephens pad. Hal said that it was acid which was spilled in the 20% range. I ask about the containment and he said that it may have found a way off. Weavertown Environmental is responding to contain the acid. I found that two tanks which had 5000 gallons of Hydrochloric acid were on site. The wrong type of valve was installed on both tanks and they failed within minutes of each other. Part of the acid was displaced down hole during the frac and approximately 1000 gals may have leaked onto the secondary containment liner. I contacted Deputy Chief Bob Worstall and advised him of the spill. I also advised him that I had discovered another chloride leak of the main containment. An area 60x30 wide has chlorides in the 1200 PPM range. Bob advised me to contact Kyle Baldwin and have him collect samples and bring more quantabs and acid stip. I collected photos and tested the fluid inside the liner and found it high in chlorides. Next I advised Hal Miller to contact a vac truck to suck up all of the  fluid out of containment so that it would not continue leaking onto the drilling pad. I spoke with Judson Shreves and Ross Kirtley to review the repairs needed and the fact that this same type of leak has happened on three drilling pads.

The flat section appears to done and the block pads are in place over both liners. The berm is being installed on the left corner of the well pad on the same side as the production equipment.

There is a new containment system on site and it appears to working as planned. The spills have been removed and the violations have been corrected.

I found that two tanks which had 5000 gallons of Hydrochloric acid were on site. The wrong type of valve was installed on both tanks and they failed within minutes of each other. Part of the acid was displaced down hole during the frac and approximately 1000 gals may have leaked onto the secondary containment liner. I contacted Deputy Chief Bob Worstall and advised him of the spill. I also advised him that I had discovered another chloride leak of the main containment. An area 60x30 wide has chlorides in the 1200 PPM range. Bob advised me to contact Kyle Baldwin and have him collect samples and bring more quantabs and acid stip. I collected photos and tested the fluid inside the liner and found it high in chlorides. Next I advised Hal Miller to contact a vac truck to suck up all of the fluid out of containment so that it would not continue leaking onto the drilling pad. I spoke with Judson Shreves and Ross Kirtley to review the repairs needed and the fact that this same type of leak has happened on three drilling pads.
1379003673 2013-09-12 I am on site today to review the clean up of the chloride spill which occurred 09/09/2013. The chlorides have been sucked up from the rock pad and also the containment liner. Part of the liner has been pulled up and the portion where the acid tanks leaked in the far right corner to the right side of the production tanks. The whole right side of the berm needs to be repaired before any further operation continues on site.
1388423312 2013-12-30 I am on site today to check on the status of the clean up of the spill which occurred over the weekend while I was off. Doug Bennett covered the spill and I am conducting a follow up inspection to see if everything has been removed and restored. I found that the material which was removed is in a roll off container still on site. The rock and soil has been removed and clean soil and rock have been put back in place. Violation has been corrected. Met with the pumper while I was on site and went over the clean up and some production numbers.
170452901 2013-12-30 On the day of my inspection the site passed preliminary restoration according to 1509.072A.
-2091846734 2013-08-08 I am on site today to see how the remediation of the site is progressing. I found that there is a containment system set up around the perimeter. Today there is a crane on site that is setting the tree on each well. Nothing further is being done on site today and there are no rigs or equipment other than the crane.
326275648 2013-08-22 I arrived today to check on the containment pad being installed. The 60 mil liner is in place and covered by a 40 mil liner. The flat section appears to done and the block pads are in place over both liners. The berm is being installed on the left corner of the well pad on the same side as the production equipment.
448190969 2013-10-22 I am on site today to check on the operation status of this well and conduct a follow up inspection for the spills which occurred onn 6/03/2013 and 9/09/2013 I found that they are drilling out plug # 14 of 17. There is a new containment system on site and it appears to working as planned. The spills have been removed and the violations have been corrected.
573529299 2013-02-22 I am on site today to witness the running and cementing of the 26 inch diameter casing string. Express Drilling has drilled a 110 ft 32 inch diameter hole. Stingray Cement Service is cementing with 280 sks of Class A cement. Cement did come up to the Tin Horn but fell back down and will need to be topped off.
-675887372 2013-09-16 The containment which was redone needs to be done over yet again. It was found to have leaks after the rebuild and Gulfport is making the containment area larger and testing for leaks after the repairs.
