0032167199 2019-02-07 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges were present on all casing strings. The 13.375-inch diameter surface casing had 50 pounds per square inch and the 9.625-inch diameter intermediate had 0 pounds per square inch. The production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. Ascent well tender was on location along with two Wise tanker trucks that were loading produce fluids from the tank battery.
0071884220 2017-05-04 Casing String Witnessed No Violations On the day inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet, hitting water at 40 feet and ran 100 feet of 30 inch 118.65 pounds per foot conductor pipe. O-Tex Pumping LLC was onsite and performed a 200 pounds per square inch pressure test, gained circulation with 2 barrels of fresh water and cemented pipe with 330 sacks Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 90 feet with 74.8 barrels of fresh water with good cement returns of 24 barrels to surface. Mix water was tested revealing chlorides of 54 parts per million, pH 7 and 59 degrees.

0796659432 2018-02-05 Completion Testing No Violations On the day of my inspection I witnessed Halliburton Services had screened of stage 51 of 80 with 75000 pounds of 100 mesh and 36000 pounds of 30/50 mesh sand put away. The 5.5 inch diameter casing was perforated with 40 shots from 12438 feet to 12306 feet. Three unsuccessful attempts have been made trying to get back into the formation with IPS coil tubing unit in-route. I met with Ascent Consultants Delbert Brown, Larry Jennings and Andy Ramoni and discussed their plans and completed a walk around.
0832317954 2017-09-19 Drill / Deepen / Reopen No Violations On the day of my inspection following up from call on 9/12/17 where it was reported that the derrick hand left the valve on the barite hopper open when he went to the rig floor resulting in a 50 barrel spill of oil based mud (OBM) on containment and 5 gallons leaving containment, I witnessed that the area had been remediated with 49 barrels and 37 gallons recovered and mixed off. When the rig has completed drilling this well all mats will be removed and any residual OBM will be removed and properly disposed. No OBM was found off the well pad with all sump drains in the closed position. Patterson UTI Drilling Rig #584 was drilling ahead on air with a 12.25 inch diameter bit at 4233 feet with a planned total depth in the intermediate section of 6700 feet. KLX Energy Services was onsite making up the shoe track for the intermediate section. Shoe track consisted of 2 joints of 9.625 inch diameter 40 pound per foot L-80 casing with a Halliburton float shoe, 1 Centek centralizer and a Halliburton float collar.
1134500937 2017-10-02 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #584 had reached a total depth in the 8.5 inch diameter lateral at 19886 feet and was tripping out of the wellbore at 16517 feet. Patterson UTI Drilling Rig #584 was performing a clean-up cycle to remove built up from the wellbore to decrease drag tripping out. I completed walk around with Ascent Consultant Alan Rice revealing 5.5 inch diameter 23 pound per foot production casing along with float equipment and centralizers were staged for the upcoming casing and cementing job.
1494890943 2017-05-15 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Drilling Rig #26 had drilled a 17.5 inch diameter hole to the depth of 1225 feet and Wyoming casing crew ran 1216 feet of 13.375 inch 61 pounds per foot J-55 casing 4.8 feet above KB. O-Tex Pumping LLC was on site and gained circulation with 5 barrels of fresh water, 35 barrels of gel, 10 barrels of fresh water and cemented casing with 1340 sacks of Class H cement with 1.5% Calcium Chloride, .2% X--<39 parts per million, pH 7 and 55 degrees.
1989158782 2018-07-06 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of nine 400 barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
