0447682700 2016-07-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that there is no current activity and the drilling permit expires on 9/16/16. The last activity was on 11/16/14 with 24 inch conductor pipe being set at 100 feet. A slip is developing along the eastside of the access road with a section of the access road eroding.
0620270123 2018-09-10 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. A gauge was present on the 9.625-inch diameter intermediate casing reading 250 pounds per square inch and 0 pounds per square inch on the 13.375-inch diameter surface casing. The production equipment consisted of five sand traps, five gas production units, three 500-barrel frac tanks, and two 400-barrel tanks. Roustabouts were on location assembling the triethylene glycol dehydration unit on the South East side of the pad beside the pipeline tie in.
0672297992 2017-07-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed H&P Flex Rig #608 had completed rig up and was picking up 17.5 inch diameter bottom hole assembly preparing to re-spud the surface section. I met with Eclipse Consultant Billy Safer completed walk around and reviewed the drilling plans for the surface section. H&P Flex Rig #608 plan to drill the 17.5 inch diameter section to 1497 feet. Upon completion 13.375 inch casing will be ran to approximately 1447 feet with cementing of casing to follow.
1057724534 2017-08-03 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P Flex Rig #608 had landed their curve at 8303 feet and was drilling ahead in the lateral with an 8.5 inch diameter on 12.0 pound per gallon mud at 10271 feet with a planned total depth of 17620 feet. I completed walk around with Eclipse Consultant Matt Hayes.
1269123912 2017-07-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed the air package and the mud tanks have been set with containment place in preparation for H&P Flex Rig #608 to arrive after drilling the lateral on the Fineran 2H. The cement has fallen on the mouse hole approximately 8 inches. I placed call to Eclipse Consultant Charlie Frazier who stated they will be re-grouted when the landing rings are installed on Wednesday.
1403661204 2017-07-25 Casing String Witnessed No Violations On the day of my inspection I witnessed that H&P Flex Rig #608 had drilled a 17.5 inch diameter hole to the depth of 1500 feet. Wyoming Casing Crew ran 581.8 feet of 13.375 inch 61 pound per foot. J-55 casing and 866.87 feet of 13.375 inch 54.5 pound per foot J-55 casing for a total of 1522.18 feet of 13.375 inch casing set at 3.7 feet above KB. KB height is 26.5 feet above ground level. Schlumberger was on site and loaded their lines with 5 barrels of fresh water and performed a low pressure test at 500 pounds per square inch and a high pressure test at 2500 pounds per square inch. Schlumberger gained circulation with 230 barrels of fresh water, 20 barrels of gel and 10 barrels of fresh water. Schlumberger cemented casing with 1150 sacks of Class A cement 15.6 pounds per gallon, 1.21 cubic foot per sack cement with 3% D154 Extender, .2% D013 Retarder, .25 pound per sack D130 Flake, .3% B160A Dispersant and .4% B348 FLAC. Cement weight was verified by pressurized mud scales and displaced with 215.5 barrels of fresh water. Cement was circulated to surface with good returns of 40 barrels, plug was landed at 590 pounds per square inch and pressured to 1170 pounds per square inch for 5 minutes and the floats held with 1 1/2 barrels back. Mix water was tested revealing chlorides of <33 parts per million, pH 7 and 68 degrees.
1496540618 2014-11-17 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Rathole Inc. had ran and set conductors on the 4H, 2H and 6H. Universal Well Services completed cementing on each string. Site inspection revealed no violations with remedial work nearly complete along the access road.
1502200089 2017-08-08 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P Flex Rig #608 had reached total depth of 17691 feet and performed clean up cycles. Wyoming Casing crew was onsite running 5.5 inch diameter 20 pound per foot P-110 casing at 10801 feet and planning on setting casing at 17644 feet. Upon completion Schlumberger will be cementing casing later this evening. I met with Eclipse Consultant Matt Hayes and reviewed the 9.625 inch diameter intermediate casing bond log revealing top of cement at 680 feet.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. A gauge was present on the 9.625-inch diameter intermediate casing reading 0 pounds per square inch and 0 pounds per square inch on the 13.375-inch diameter surface casing. The production equipment consisted of five sand traps, five gas production units, three frac tanks, two 400-barrel tanks, and a triethylene glycol dehydration unit. Identification was located on the tank battery. Produced water was being separated here and stored on pad. The produced oil and produced natural gas were being transported to the Outlaw pad for gathering via pipeline.
