| On the day of my inspection I found that Nomac Drilling rig 80 had reached a total depth of 15985'. They were in the process of running new 5.5 production casing. AEU reported that a spill had just occurred prior to my arrival. They estimated that 20 gallons of oil based mud had been released from a CurryVac truck. The mud was released outside of containment. No fluid made it off of the drilling pad. The spill had been cleaned up by the time I was notified. The contaminated gravel had been scraped up and sent to the landfill. I did not observe any residual contamination at the spill site.

| On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I witnessed Precision Rathole Inc. drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65#/ft. conductor pipe. O-Tex Pumping LLC was on site and gained circulation with 5bbls of fresh water, 15bbls of gel and 5bbls of fresh water. | On the date of inspection I was informed by American Energy Utica (AEU) consultant Van Dietrich that the Nomac 80 rig had drilled a 17.5 inch diameter hole to a depth of 970 feet. Wyoming Casing Crew set 13.375 inch 54.50lbs/ft casing on bottom. O-Tex Pumping, LLC pumped 1270sk of Class H cement, 15.6ppg, 1.2 yield, 2% CaCl, 0.25lbs/sk flake, 0.4% C-41P. O-Tex released the plug and approximately 2bbl into displacement the pressure spiked to 625psi. O-Tex stopped pumping and bled off pressure but still continued to pressure up. AEU plans to drill out the plug and cement, run a Cement Bond Log to find the top of cement, and then decide how to bring cement to surface from there.

| On the day of my inspection I found that Nomac Drilling rig 80 had new 9.625 casing ran to a depth of 6486'. The casing was equipped with a DV tool at 3516'. They were circulating when I arrived. O-Tex Pumping attempted to cement the casing to surface in one stage. Although good circulation was maintained throughout the job, no cement was circulated to surface. There were approximately 3 barrels of gel spacer circulated to surface when O-Tex reached their maximum displacement volume. Nomac 80 opened the DV tool and circulated for 12 hours. O-Tex cemented the second stage of the job. Good circulation was maintained throughout the job with approximately 80 barrels of cement returns.

| On the day of my inspection I found that Nomac Drilling rig 80 had reached a total depth of 15985. They were in the process of running new 5.5 production casing. AEU reported that a spill had just occurred prior to my arrival. They estimated that 20 gallons of oil based mud had been released from a CurryVac truck. The mud was released outside of containment. No fluid made it off of the drilling pad. The spill had been cleaned up by the time I was notified. The contaminated gravel had been scraped up and sent to the landfill. I did not observe any residual contamination at the spill site.

| The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with open valves. The production equipment on location consisted on seven sand traps, seven gas production units, three horizontal heater treaters, four compressors, six 400-barrel tanks, five 500-barrel tanks, two vapor recovery towers, and one combustor. The four recently completed wells on location were flowing back. The flow back equipment on location consisted of three gas buster tanks, one frac tank, and three horizontal line heaters.

| On the day of my inspection I found that Nomac Drilling rig 80 had new 9.625 casing ran to a depth of 6486. The casing was equipped with a DV tool at 3516. They were circulating when I arrived. O-Tex Pumping attempted to cement the casing to surface in one stage. Although good circulation was maintained throughout the job, no cement was circulated to surface. There were approximately 3 barrels of gel spacer circulated to surface when O-Tex reached their maximum displacement volume. Nomac 80 opened the DV tool and circulated for 12 hours. O-Tex cemented the second stage of the job. Good circulation was maintained throughout the job with approximately 80 barrels of cement returns.

| On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I witnessed Precision Rathole Inc. drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65#/ft. conductor pipe. O-Tex Pumping LLC was on site and gained circulation with 5bbls of fresh water, 15bbls of gel and 5bbls of fresh water. O--Flake. Cement returns came back early with returns of 34bbls and cement stayed surface. I Instructed AEU Drilling Superintendent to notify ODNR prior to re-spud so verification of top of cement inside conductor can be seen and if any remedial work will be necessary.

| On the date of inspection I was informed by American Energy Utica (AEU) consultant Van Dietrich that the Nomac 80 rig had drilled a 17.5 inch diameter hole to a depth of 970 feet. Wyoming Casing Crew set 13.375 inch 54.50lbs/ft casing on bottom. O-Tex Pumping, LLC pumped 1270sk of Class H cement, 15.6ppg, 1.2 yield, 2% CaCl, 0.25lbs/sk flake, 0.4% C-41P. O-Tex released the plug and approximately 2bbl into displacement the pressure spiked to 625psi. O-Tex stopped pumping and bled off pressure but still continued to pressure up. AEU plans to drill out the plug and cement, run a Cement Bond Log to find the top of cement, and then decide how to bring cement to surface from there.

| The purpose of my inspection was to witness the status of temporary abandonment on well #2-4341.  Guernsey County Inspector Austin Gutridge was present during the inspection.  While on location we met with Ascent Drilling Superintendent James McGee. At the time of inspection, Patterson #577 was on location drilling on well Red Hill Farm 10H- #2-4526.  The drilling crew was making a trip to change the drilling assembly.  Well 5H was temporarily abandoned.  The production tree was removed and night cap was in place.  Annular valve had pressure gauges installed for monitoring.  Pressure transducers were installed on the night cap monitoring port to allow for constant monitoring with alarms.  A steel cage was located over the well head.  A total of three (3) wells were temporarily abandoned.  The surface production equipment had been removed.  The transfer point was isolated.  No simultaneous operations or anti-collision issues were identified during the inspection.

They were in the process of running new 5.5 production casing. AEU reported that a spill had just occurred prior to my arrival. They estimated that 20 gallons of oil based mud had been released from a CurryVac truck. The mud was released outside of containment. No fluid made it off of the drilling pad. The spill had been cleaned up by the time I was notified. The contaminated gravel had been scraped up and sent to the landfill. I did not observe any residual contamination at the spill site.

0658550140 2016-12-14 Production Wells No Violations On the day of my inspection I witnessed the ground was snow covered and that the well is online and equipped with a Universal Wellhead LLC casing and production wellhead along with a Universal Wellhead LLC production tree with valves in the open position. Casing pressure was at 600 pounds per square inch and the tubing was at 480 pounds per square inch with annular pressure monitoring in place. Production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 2-NGL tanks, 2 compressor units, a combustor unit and a total of 12-400 barrel tanks all within containment and shared with the 1H and 3H.

During my inspection the well was equipped with a production tree style wellhead with open valves. The production equipment on location consisted on seven sand traps, seven gas production units, three horizontal heater treaters, four compressors, six 400-barrel tanks, five 500-barrel tanks, two vapor recovery towers, and one combustor. The four recently completed wells on location were flowing back. The flow back equipment on location consisted of three gas buster tanks, one frac tank, and three horizontal line heaters.
0975637734 2014-11-05 Casing String Witnessed No Violations On the day of my inspection I found that Nomac Drilling rig 80 had new 9.625 casing ran to a depth of 6486. The casing was equipped with a DV tool at 3516. They were circulating when I arrived. O-Tex Pumping attempted to cement the casing to surface in one stage. Although good circulation was maintained throughout the job, no cement was circulated to surface. There were approximately 3 barrels of gel spacer circulated to surface when O-Tex reached their maximum displacement volume. Nomac 80 opened the DV tool and circulated for 12 hours. O-Tex cemented the second stage of the job. Good circulation was maintained throughout the job with approximately 80 barrels of cement returns.

During my inspection the well was equipped with a production tree style wellhead with open valves. Gauges present read 0 pounds per square inch on the 13.375-inch diameter surface casing, 0 pounds per square inch on the 9.625-inch diameter intermediate casing, 1500 pounds per square inch on the 5.5-inch diameter production casing, 400 pounds per square inch on the production tubing. The production equipment on location consisted on seven sand traps, seven gas production units, three horizontal heater treaters, four compressors, six 400-barrel tanks, five 500-barrel tanks, two vapor recovery towers, and one combustor.

The lubricator has been removed and there is no pressure at surface. The intermediate casing is locked out and tagged out. Cage has been placed around the well.
1403786729 2014-06-26 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I witnessed Precision Rathole Inc. drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65#/ft. conductor pipe. O-Tex Pumping LLC was on site and gained circulation with 5bbls of fresh water, 15bbls of gel and 5bbls of fresh water. O--Flake. Cement returns came back early with returns of 34bbls and cement stayed surface. I Instructed AEU Drilling Superintendent to notify ODNR prior to re-spud so verification of top of cement inside conductor can be seen and if any remedial work will be necessary.
1414428206 2014-10-27 Casing String Witnessed No Violations On the date of inspection I was informed by American Energy Utica (AEU) consultant Van Dietrich that the Nomac 80 rig had drilled a 17.5 inch diameter hole to a depth of 970 feet. Wyoming Casing Crew set 13.375 inch 54.50lbs/ft casing on bottom. O-Tex Pumping, LLC pumped 1270sk of Class H cement, 15.6ppg, 1.2 yield, 2% CaCl, 0.25lbs/sk flake, 0.4% C-41P. O-Tex released the plug and approximately 2bbl into displacement the pressure spiked to 625psi. O-Tex stopped pumping and bled off pressure but still continued to pressure up. AEU plans to drill out the plug and cement, run a Cement Bond Log to find the top of cement, and then decide how to bring cement to surface from there.

Accompanied by ODNR Kenn Frye, during our inspection we completed a walk around with Ascent Resources Superintendent James McGee. Patterson 577 was on location in the process of drilling the 10H. This well had been decommissioned prior to rig Patterson 577 moving onto location. The well was equipped with valves on all casing strings. All valves showed 0 pounds per square inch and the a cage was placed over the well.

Production equipment included a sand trap, a GP Unit, a line heater, a vapor recovery system, 2-NGL tanks, 2 compressor units, a combustor unit and a total of 12-400bbl tanks all within containment and shared with the 1Hand 3H. The well has been online for two weeks and current production levels are 800bbls of oil, 168bbls of water and 2MMCF of gas per day.
1974025980 2018-09-25 Production Wells No Violations The purpose of my inspection was to witness the status of temporary abandonment on well #2-4341. Guernsey County Inspector Austin Gutridge was present during the inspection. While on location we met with Ascent Drilling Superintendent James McGee. At the time of inspection, Patterson #577 was on location drilling on well Red Hill Farm 10H- #2-4526. The drilling crew was making a trip to change the drilling assembly. Well 5H was temporarily abandoned. The production tree was removed and night cap was in place. Annular valve had pressure gauges installed for monitoring. Pressure transducers were installed on the night cap monitoring port to allow for constant monitoring with alarms. A steel cage was located over the well head. A total of three (3) wells were temporarily abandoned. The surface production equipment had been removed. The transfer point was isolated. No simultaneous operations or anti-collision issues were identified during the inspection.

Annular pressure monitoring was present and the well was caged. Four additional wells on pad had just finished hydraulic stimulation operations and the frac equipment was demobilizing off pad.

The production equipment included 12-400 barrel tanks within containment, 2-NGL tanks, a vapor recovery tower, 2-compressor units, 2-horizontal heater treaters, 2-oil stabilizer units and a gas combustor all shared with the 1H and 3H. Also present a GP Unit and a sand trap with identification present and production records current.
