0001495886 2016-05-09 Final Restoration Violations Noted On the day of my inspection accompanied by ODNR Southeast Regional Supervisor Brent Bear, I witnessed the restoration of the site was not completed with the access road not restored, number two limestone scattered over the location, several low spots and no erosion control measures in place. I placed a call to Eclipse EHS Field Representatives on 3/21/16 advising them that I needed an update on when the restoration would be completed. I received return calls on 3/22/16 and 4/11/16 from representatives stating the restoration would be completed prior to the due date of 5/5/16. Owner/operator shall complete restoration according to ORC 1509.072B, restoring access road, removing limestone from location, restoring grade and applying erosion control measures where needed.
0067432714 2014-06-11 Preliminary Restoration No Violations On the day of my inspection I witnessed that the production equipment included 2-100bbl tanks, a 30bbl brine tank, a vertical separator, a drip and a 1.5 inch Honeywell rotary meter with a chart recorder all within an earthen dike. The well was equipped with 7 inch casing, 4.5 inch casing, 2.375 inch tubing, rods and a pumping unit powered by natural gas all valves were closed at the wellhead. Salesline has not been installed. Well identification was present and the site passed preliminary restoration according to 1509.072A.
0159881020 2016-06-09 Final Restoration No Violations On the day of my inspection following up on Compliance Notice-1495886, I met with Eclipse Resources EHS Superintendent Bruce Carpenter at the site as the site now meets ORC 1509.072B with a land owner waiver for the access road left in place for farm use.
0206579875 2015-11-05 Plug / Plug Back No Violations On the day of my inspection I witnessed the casing pressure was at 25psi and Wrights Well Service opened well and it began flowing back. WWS tripped inside the 4.5 inch casing with their tools and tagged the last plug at 2045 feet. WWS tripped 2.375 inch tubing to 2044 feet. Formation Cementing Services was onsite and pumped 30bbls bottoms up of 9.5ppg brine to kill the well and then pumped a 35 sack 465 foot Class A plug from 2044 feet to 1616 feet. Cement was displaced with 6.3bbls of 9.5ppg brine water. WWS tripped 500 feet of 2.375 inch tubing and Formation loaded tubing and gained circulation. WWS waited 30 minutes and the well remained static. WWS tripped 2.375 inch tubing out and ripped 4.5 inch casing at 1138 feet. Formation pumped a 40 sack 200 foot Class A plug from 1094 feet to 894 feet. Cement was displaced with 14.3bbls of water. WWS tripped 4.5 inch casing to 200 feet and Formation pumped a 40 sack 200 foot Class A surface plug from 200 feet to surface. Cement stayed at the surface.
0409127984 2015-11-02 Plug / Plug Back No Violations On the day of my inspection I witnessed that the Wrights Well Service was onsite and tripped 2.375 inch tubing out from well, removing seating nipple and mud anchor. WWS tripped 2.375 inch tubing back in to a depth of 4240 feet and Formation Cementing Service was onsite gaining circulation and pumping 51.5bbls of gel followed by a 90 sack 500 foot bottom plug from 4240 feet to 3740 feet. Cement was displaced with 5.3bbls of gel and 8bblsof fresh water. WWS tripped 2.375 inch tubing to 2456 feet and Formation gained circulation and pumped a 26 sack 200 foot isolation plug from 2456 feet to 2256 feet. Cement was displaced with 8.7bbls of water and WWS tripped tubing out of cement and will tag plug tomorrow before completing plugging.

0684786799 2015-11-03 Plug / Plug Back No Violations On the day of my inspection I witnessed the well had pressure and Wrights Well Service opened the 4.5 inch casing as it was bleeding down the well came on fluid. The 2.375 inch tubing had no pressure on it. Formation Cementing Service is in route to kill well. Upon return Formation had pumped on well at 4bbls/min with a pressure of 500psi not circulating on well. The well was shut-in with 300psi on the casing. I placed call to Tim Altier and requested a plan as to how they were going to kill the well. Mr. Altier stated that the plan was to pump Clinton brine to kill well and I stated why are you not circulating on the well? Mr. Altier stated that they could kill the well by pumping on it. I stated that they were to circulate on the well and he agreed to put a choke on the casing and displace the lighter brine with the Clinton brine.

0925892318 2016-06-02 Final Restoration No Violations On the day of my inspection following up on Compliance Notice-1495886, I met with Eclipse Resources EHS Superintendent Bruce Carpenter at the site and reviewed the restoration plans which will begin on 6/6/16. The land owner will be waiving the access road and Eclipse will be providing a land owner waiver.
1065779818 2015-11-04 Plug / Plug Back No Violations On the day of my inspection I witnessed the casing pressure was at 150psi and Eclipse Resources installed a choke and opened casing up. Formation Cementing Services was onsite and tripped inside the 2.375 inch tubing with their slick wire line and tagged the packer plug at 2103 feet. Formation pumped 40bbls of 10# brine at 2bbls/min to kill the well. Wright Well Service tripped in 3 joints of 2.375 inch tubing and Formation pumped a 35 sack 465 foot Class A plug from 2081 feet to 1616 feet. Cement was displaced with 6.3bbls of water. WWS tripped 2.375 inch tubing out as the last joint was pulled the well began flowing back. Operator was instructed to shut well in until tomorrow.
