Operator broke circulation and ran 145 sack of class A 3% CaCl. Good return to surface of cement. Pits are in good repair. This is a skid rig surface from the original permit 1970.

1195142732 2007-11-07 Drill / Deepen / Reopen No Violations On site for surface cement job. Operator had set two joints of conductor pipe, 11.75 inch pipe. When operator was running 8.625 pipe, operator was hitting a solid object down hole. Operator was not sure if conductor pipe had unscrewed or if they were having a problem with a rock down hole. Operator decided to pull surface pipe and clean up the hole with the bit. Once surface pipe was pulled from the hole, it was found that the last section of pipe had fallen off. Operator was planning on fishing the 8.625 section of pipe out of the hole. The night of 11/08/2007, operator called me to inform me that they could not get the pipe out of the hole. The maximum depth that they could reach was 96 feet. The operator wanted to plug the well and skid the rig. I informed Vic from Capstar that they could plug the hole as long as they got below the Berea. Plugging from 96 feet to surface wouldnt not be sufficient to protect the ground water. Vic from Capstar called me in the morning of 11/09/2007 to inform me that they had gotten past the blockage. I instructed Vic to plug from 50 below the Berea to surface. I was not notified for plugging of the hole. Operator set plugs 343 to 243 across the Berea and 150 feet to surface. Spoke with Dave from Excalibur, I informed him to skid rig he would need to send to the Columbus office immediately a new permit application, plat and fee.
