Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0048910483 2013-03-28 Production Wells No Violations My inspection found the well to be equipped with a pump jack. The annulus valve was closed. The well and tank battery are in good repair. The tank battery consists of a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery and the pump jack. The lease road is in stable condition. No violations found.
0448848251 2017-12-14 Production Wells No Violations At the time of this inspection, the well was equipped with a pumpjack and the annulus and flow line valves were open. Production records for this well were current. The tank battery consisted of one 210 BBL production tank, one 100 BBL brine tank and a mechanical separator. Valves on the separator were open and the chart showed production. The tank battery was diked and had legible signage. The lease access was stable. No violations observed.
0480051918 2011-08-25 Preliminary Restoration No Violations Site has been graded and seeded. Erosion south of the pump jack needs seeded. There is rill erosion approximately 20 feet in length, sedimentation is on location. Rill needs repaired. There is paraffin under the pump jack approximately 5 gallons. Paraffin needs removed from location. Recently repaired area behind needs repair seeded to prevent erosion. Lease road is stable Tank battery is in good repair.
0522004284 2011-07-26 Preliminary Restoration No Violations There has been crude oil spilled in the battery. Stone inside the battery is stained. Spill by separator is approximately 1.5 feet in diameter. Spill along the flow line from separator to the production tank is a few inches wide and 3 feet long. Stone needs mixed with bio remediation material or removed. Affected area behind the tank battery needs seeded to prevent erosion. Erosion south of the well head has stabilized. Affected area from drilling, area north, west and east of the wellhead vegetation is starting to become established. Affected area south and south east of the wellhead the vegetation is sparse. Erosion has channel has stabilized, but the channel needs repaired. There is oil spray behind the wellhead. Oil spray needs remediated. Tank battery has a 100Bbl, 200 Bbl and mechincal separator in it.
1194273507 2007-11-05 Drill / Deepen / Reopen No Violations Operator is digging the blow pit. Pit is in clay. No rock noted in the strata of the pit. Operator is drilling on air and soap at the time of inspection, drill depth is 105. Well is making water, driller is going to switch over to fluid next drill pipe. Operator was using a defoaming agent to keep the soap down. Defoaming agent was Off road diesel fuel, sprinkled into the soap. Defoaming agent worked. Blow pipe was leaking at two locations. Leaks were at the discharge point and in the middle, both leaks repaired at the time of inspection. Lease road is stable, called Dave from Excalibur asked him to make sure he has equipment on site to clean the roads, rains is predicted for tonight. There is rill erosion south of the Pump jack. Area approximately 180 X 150 is barren, remainder of the site has a covering of native vegetation on it. Tank battery has one 100BBL tank and one 210 BBL tank and a mechanical separator. Barren area needs ruffed up, seeded and mulched to prevent of site erosion. Called John 330-472-2046 asked that the area be seeded and mulched. Gave him a verbal NOV regarding reclamation. Well was spudded 11/04/2007.
