Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1177930132 2012-09-13 I went to the site to review the construction process of the drilling pad. The pad was excavated, and the berms up. Preliminary soil stabilization has been completed. No stone in place at this time. There is a gate at the access road, but no fence around the pad yet.
1365613966 2012-08-07 I went to the site and conducted a pre-construction site inspection with Brett Bell, Chesapeake Field Representative.
1365617476 2012-12-07 I went to the site to witness the cementing of the surface casing. The drilling contractor, Nomac 310, spudded the well on 12/05/2012. The 17.50' diameter surface hole was drilled to 715
1460173683 2012-10-17 I went to the site to check the status of the cellar floor. Jimmy Freeman of Chesapeake indicated that it would be completed with redi-mix the next day.
-1601422631 2012-12-04 I went to the site and had a pre-spud meeting with drilling consultants Mike Kirkpatrick and Gene Belsom. Nomac 310 hopes to commence drilling on 12/05/2012.
-1887554950 2012-10-15 I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 120'. Express installed 120' of 20 OD casing. During drilling, water was encountered at an approximate depth of between 55 and 60'. Allied Cementing Co., LLC was the cementing contractor. The cement was mudscaled at 16.2 pounds per gallon (ppg), and adjusted to 15.6 ppg. Allied pumped 47 bbls of cement slurry at 15.6 ppg. The cement was displaced with 40 bbls of water. Cement returns to surface were observed. An estimated 10 bbls of cement returned to surface. Displacement was completed at 8:47pm.
-1889816396 2013-07-02 I reviewed the site to assess the completion of preliminary restoration. There is a sign board at the access road entry featuring the well name, permit number and emergency contact information. The access road has a padlocked gate near Winona Road. The access road was in good repair, and drainage and erosion controls were in place and appear to be functioning. Ground cover was well established along the access road. I found the drilling/production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. The pad has a crushed limestone surface, and is in good repair. Along the interior of the chain link fence there are two site drains installed
312237210 2012-10-15 I was on site for the cementing of the conductor for this well location with Rick Anderson. I witnessed cement come back to surface. The pad is in good condition and there are no violations at this time.
-441305200 2013-04-09 I went to the site for a status check and found that the site was being prepared for well stimulation. A number of frac tanks were present on site, and a water pumping set-up was present on Depot Road where it intersects Cold Run Creek. A crew was installing a security camera. No one else was present.
-562621678 2012-12-12 I went to the site to witness the cementing of the intermediate casing. The drilling contractor, Nomac 310, drilled a 12.25 diameter hole to 1800
599667445 2012-12-08 I was at the site to witness the top-off cement job on the surface casing. Since circulation was lost on the primary surface string cement job, a cement bond log (CBL) was run to determine the depth of the top of the surface casing cement. The CBL indicated that the cement top was at 362
-730818307 2012-10-14 I was at the site to check the progress on the conductor drilling operation. Express Energy Services was on location and in the process of drilling the conductor hole. The rig was drilling at 100
-920189640 2012-10-24 While enroute to other inspections, I stopped at the site to collect GPS data for the wellhead and access road.
9471531 2012-12-26 I went to the site to witness the cementing of the production casing. The drilling contractor, Nomac 310, drilled an 8.75 diameter hole from beneath the base of the intermediate casing at 1800
