-Tex Pumping LLC was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement job was done in stages. Following two spacers designed to recover the oil-based mud used to drill the hole and condition the hole, respectively, the tail cement slurry was pumped. O-Tex pumped 443 bbls of tail cement at 15.3 pounds per gallon (ppg). The plug was dropped, and displaced with 279 bbls of water. The plug bumped the float collar at 11:37pm on 12/26/2012. Pressure held. The float held when the pressure was released, and 3.0 bbls of water bled back.

0312237210 2012-10-15 Drill / Deepen / Reopen No Violations I was on site for the cementing of the conductor for this well location with Rick Anderson. I witnessed cement come back to surface. The pad is in good condition and there are no violations at this time.

A number of frac tanks were present on site, and a water pumping set-up was present on Depot Road where it intersects Cold Run Creek. A crew was installing a security camera. No one else was present.

-Tex Pumping LLC was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.5 pounds per gallon (ppg), and adjusted to 15.6 ppg. O-Tex pumped 146 bbls of cement slurry at 15.6 ppg. Cement returns to the surface were observed. An estimated 13 bbls of cement returned to the surface. Cement returns were mudscaled at 13.0 ppg. The plug was displaced with 133 bbls of water. During the plug displacement, the plug did not bump the float collar when the calculated displacement plus half the volume of the shoe track was reached, so pumping was discontinued. The displacement was completed at 11:36am on 12/12/2012. The float held when the pressure was released and 0.5 bbls of water bled back.

0599667445 2012-12-08 Drill / Deepen / Reopen No Violations I was at the site to witness the top--Tex Pumping LLC was the cementing contractor. O-Tex mixed and mudscaled the cement, and pumped 57 bbls of cement slurry at 15.6 pounds per gallon (ppg). Cement returns to the surface were observed. Approximately 3 bbls of cement returned to the surface. Following a 2.5 hour wait, the cement top had fallen approximately 29.5 feet. O-Tex pumped an additional 6 bbls of cement to bring the cement back to the surface. The job was complete at 1:05pm on 12/08/2012.

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. There is also two compressors on this pad that is used for vapor recover from the production tanks. Along the interior of the chain link fence there is a site drain installed. Outlets are located at the east and west sides of the pad. Inside the fence, there is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete and approved.

An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #8H well is in production. The well is equipped with a plunger lift assembly. There is a steel dike around the tank battery.

1177930132 2012-09-13 Surface Facility Construction No Violations I went to the site to review the construction process of the drilling pad. The pad was excavated, and the berms up. Preliminary soil stabilization has been completed. No stone in place at this time. There is a gate at the access road, but no fence around the pad yet.
1293463694 2017-02-09 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1750-00-00. RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. At the time of my inspection, the local area had received a coating of snow that partially covered the location. The lease road was drivable. No signs of erosion were observed adjacent to the road traveling back to the pad. The soil stockpile remains stable with well-

-Tex Pumping LLC was the cementing contractor. There was intermittent circulation during the cementing operation. O-Tex pumped 25 bbls of gel with Bero-sel ahead of the cement. The cement was mudscaled and pumped at 15.6 pounds per gallon (ppg). O-Tex pumped 177 bbls of cement slurry at 15.6 ppg. Circulation was restored when 45 bbls of the cement had been pumped. Circulation was lost again when 80 bbls of the cement had been pumped. Circulation returned when 156 bbls of cement had been pumped. Circulation continued until about 90 bbls into the displacement, when it stopped for the duration of the job. No cement returns to surface were observed. The plug was displaced with 101 bbls of water. The plug bumped the float collar at 5:52pm. O-Tex pressured up on the casing, and the pressure held. When the pressure was released, approximately 1.0 bbl of water flowed back. Since circulation was lost and there were no cement returns to surface, a bond log will be run to determine the depth of the top of the cement.

1887554950 2012-10-15 Drill / Deepen / Reopen No Violations I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 120. Express installed 120 of 20 OD casing. During drilling, water was encountered at an approximate depth of between 55 and 60. Allied Cementing Co., LLC was the cementing contractor. The cement was mudscaled at 16.2 pounds per gallon (ppg), and adjusted to 15.6 ppg. Allied pumped 47 bbls of cement slurry at 15.6 ppg. The cement was displaced with 40 bbls of water. Cement returns to surface were observed. An estimated 10 bbls of cement returned to surface. Displacement was completed at 8:47pm.

A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The well has a frac valve in place, but there is no production equipment at the site. The cellar is full water to the top. Preliminary restoration approval will not be finalized pending installation of production equipment.
