I received the complaint on 12/13/2018 from caller who wished to remain anonymous and was concerned of dozer work at the Shaw Properties freshwater impoundment causing stability issues of the pad slope. This freshwater impoundment is located west of the Shaw Properties producing pad. DOGRM Engineer Paul Logan was on site on 12/14/2018 to access the location. He spoke to a contractor who was on site doing dozer work. The contractor was working for the landowner and was removing soil to fill in a wet area located at the base of the west slope of the pad. The contractor stated that no more than two feet of soil has been removed. The DOGRM engineer observed no stability issues on 12/14/2018. I located the disturbed area on 12/20/2018, and I observed no erosional and stability issues or concerns. The complaint can be closed at this time. |

Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:
Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed. |
1378930923 | 2013-09-05 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 9 5/8 inch intermediate casing and to obtain the cement information from the 13 3/8 inch surface casing job on 9/02/13 (Labor Day Holiday). My inspection revealed that Patterson Drilling Rig # 329 drilled the 17 1/2 surface hole to a total depth of 854 feet and ran 840 feet of 13 3/8 inch surface casing (54.5 pound per foot and 12.615 inside diameter). Seven bowspring centralizers were installed on the casing string according to the casing tally. OTEX cemented the casing with 780 sacks of Class H 2% CaCl, 0.4% C-41P, and 1/4 pound per sack cello-flake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.25 gallons of mix water per sack. OTEX pumped 166.7 barrels of cement slurry, 124 barrels of freshwater displacement, and got 39 barrels of cement returns to surface. OTEX bumped the plug on 9/02/13 at 09:10 pm with 350 PSI and then increased the pressure to 1170 PSI to conduct the casing test (approved according to cement ticket) NOTE: The surface job was not witnessed due to Holiday. Todays inspection revealed that Patterson Drilling Rig# 329 has drilled the 12 1/4 inch hole to a total depth of 2140 feet and ran 2125 feet of 9 5/8 inch casing (40# pound per foot and inside diameter is 8.830). There were 16 bowspring centralizers installed on the casing string (on joints 1,3,6,9,12,15,18,21,24,27,30,33,36,39,42, and 45). OTEX cemented the 9 5/8 intermediate casing with a lead cement of 140 sacks (30.92 barrels of cement slurry) Class H 10% NaCl, 0.4% C-41P, 0.1% C-35, and 1/4 pound per sack cello-flake. The cement had a density of 15.6 pounds per sack, 1.24 cubic feet per sack, and used 5.48 gallons of mix water per sack. OTEX pumped 620 sacks (133.61 barrels) of Class H 3% CaCl, 0.4% C-41P,0.1% C-35, and 1/4 pound per sack cello-flake tail cement. The cement had a density of 15.6 pounds per gallon, a yield of 1.21 cubic feet per sack, and used 5.25 gallons of mix water |
