0414652417 2018-03-22 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains four 400-barrel production fluid holding tanks. There were also three 500-barrrel frac tanks on site. Annular pressures read on the gauges provided appeared to be 0 pounds on between the outside of the 9.625-inch diameter casing and the inside of the 13.375-inch diameter casing and 625 pounds between the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing . The production records within RBDMS are currently updated to 2017.

0996927703 2018-08-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a 3-string casing design with 5.5-inch diameter production casing that appeared to be open flowing with no artificial lift technology. Annular pressure monitoring was in place for the 9.625-inch diameter intermediate casing gauge read 1100 pounds per square inch, and the 13.375-inch diameter casing gauge read 620 pounds per square inch. Production equipment included a Sand Separator, Gas Processing unit and four 400-barrel production fluid tanks, and a 50-barrel produced water tank shared with the other producing wells on the well pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
1145019081 2018-08-07 Production Wells No Violations Status check inspection found the well pad surface and lease road has a gravel surface and is in good repair. There are 6 wells present on the pad all in production. The said well is equipped with a lower A section and upper B section with 2 diameter riser pipes present at the well head and equipped with valves and manual pressure gauges to monitor annular pressure. The 13.375 x 9.625 diameter casing pressure was 600 pounds per square inch (psi) the 9.625 x 5.5 diameter casing pressure was 800 psi. The 5.5 diameter production casing internal working pressure was 2,000 psi and the production flow line pressure was 1,100 psi. The well is equipped with production tubing and was free flowing above line pressure. There are 6 gas processing units on location. At the right rear corner of the well pad is the production fluid facility consisting of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. There is also a midstream station at the right front corner entry point of the well pad. Fire extinguishers and emergency shut down control points are present at all the critical areas of the well pad. The well pad perimeter is protected by an earthen berm. Also on this day I contacted Gulfports Production Superintendent Robert Coots and we discussed the 9.625 annular pressures on several of the wells on the pad along with having the well pad annulis monitored more closely while stimulation operations are going on the neighboring well pad.

-55 grade casing to a measured depth of 1,681. Stingray Cement Service cemented the casing by first pumping a 25 barrel 6% gel space followed by a 310 barrel class A cement mix consisting of 1,453 sacks of cement with 1% calcium and .25 pound per sack celluseal, the cement weight was verified at 15.6 pound per gallon by weight of scale. The cement capacity was pumped and the plug dropped. During displacement at 131 barrels into displacement the pressure flow line between the cement truck and well head leaked. The crews worked 17 minuets to repair the line and when the cement company attempted to re establish pumping high pump pressure of 3000 psi was experienced. And no further circulation could be achieved. Calculations indicate the top of cement in perfect hole conditions would be at 197. The company ran 1 tubing and tagged the top of cement at 194 measured depth and then topped the annular side off with a 75 barrel cement slurry mix with a reported 32 barrels of cement returns to surface.
