Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0051549742 2019-04-09 Production Wells No Violations Status check inspection found the well currently shut in while additional wells are drilled on the well pad. The A section pressure is 2.8 pounds and the B section pressure is 520 pounds pressure. 3 wells were pre existing and 2 wells are being drilled. A 3 pre existing wells are shut in.
0539197665 2019-05-30 Production Wells No Violations On the day of my inspection I witnessed that Patterson UTI 292 had been moved off location, the well was currently still shut in and under simultaneous operations. The well was equipped with 5.5-inch diameter production casing, 2.875-inch diameter tubing that appeared to be open flowing. There was annular pressure monitoring in place using transducers with digital readouts, the 13.375-inch diameter first intermediate casing string pressure was at 15.5 pounds per square inch, the 9.625-inch diameter casing pressure was at 530 pounds per square inch. Production equipment included an individual gas processing unit, and three 400-barrel steel production fluid storage tanks. There was also a midstream valve site with glycol injection system on the northwest corner of the pad. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was current.
0746968022 2014-10-21 Casing String Witnessed No Violations Falcon 38 drilled a 24x446 open hole. Previous casing set was 26x100 conductor. Rig ran 20x417 of surface casing (94#/ft). Casing was centralized with 5 centralizers and a basket was set at 373. Company man said they did not lose circulation while drilling. Baker-Hughes is onsite with 473 sks of class A (15.6ppg; 1.21 yield). 30% excess ran. Cement additives include 2% CaCl and .25#/sk celloflake. 136 bbl freshwater used to displace. 20 bbl of cement returns to surface.
0833152912 2017-09-07 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing without any artificial lift technology present at this time. The well is piped to a gas processing unit and then to three 400 barrel production fluid holding tanks. Production records in RBDMS are current to 2017.
1182389143 2019-03-04 Production Wells No Violations Status check inspection found (3) wells present on the pad with all (3) wells completed and in production. The well head assembly consists of a lower A and upper B section with 2 diameter riser pipes attached to the outlet ports on each section. The riser pipes are equipped with valves and gauges for the purpose of annular pressure monitoring. The A section riser pipe pressure indicates 536 pounds per square inch pressure (psi), the B section indicates 244 psi. The well head assembly consists of a production tree equipped with (3) gate valves followed by an emergency shut off valve. The well feeds into a gas processing unit supported by a production fluid facility that consists of (3) steel 400 barrel fluid volume tanks situated inside a polymer lined steel containment dike area. Identification is posted at the well head and lease road entry point with hazard warning labels on the tank battery. There is a 3rd party midstream valve station situated at the center rear area of the well pad.
1434858952 2018-07-23 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 5.5-inch diameter production casing, and 2.875 inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 495 pounds per square inch, the 13.375-inch diameter surface casing gauge read 115 pounds per square inch. Production equipment included an individual Gas Processing unit and 3- 400-barrel production fluid tanks shared with 34013207980000, 34013207990000. The well pad and all equipment were contained in an earthen dike with secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tags were present, and production was up to date.
1464631594 2015-01-21 Completion Testing No Violations Call from the operator advising that they plan to mobilize completion operations to this site. Current plan is to drill out the toe and begin stimulation work within the next three weeks. This site will utilize a zipper frac which indicates stimulating all three wells simultaneously.
1591918203 2016-10-04 Production Wells No Violations This well is currently equipped properly to be considered capable of production. The piping from the well leads to a gas processing unit and then to the tank battery. The tank battery consists of three 400 barrel production fluid tanks. The site appears to be in satisfactory condition.
1693279515 2015-01-21 Preliminary Restoration No Violations The preliminary restoration for this well is completed. The pad is in satisfactory condition with proper erosion and sedimentation controls in place. Company shall continue to monitor the location to ensure the pad remains in satisfactory condition. Preliminary restoration passed.
2099197430 2014-10-27 Casing String Witnessed No Violations Falcon 38 drilled a 17.5x2110 open hole and set 13.365x2097 casing (54.4#/ft). Previous casing set was 20x417. Baker hughes is onsite to cement the casing with 1722 sks of class A (15.6ppg; 1.20 yield) cement. 30% excess was ran in open hole and cement additives were .25#/sk celloflake. 335 Bbl of water used for displacement and 23 bbl of cement returned to surface.
2136003188 2014-07-10 Casing String Witnessed No Violations Well Services group augered an open hole (7-9-14) with a depth of 100 and a diameter of 30. They also set 100 of 26 (94#/ft) conductor pipe. Allied oil and gas services, LLC was onsite to cement casing in place using a class A cement with 2%CaCl and .25#/sk floseal. Returns were viewed in cellar and 13 barrels of cement were returned to surface.
