Pump cement though 2467 ft of used 2.375 inch tubing stung into liner hanger. Use latch down wiper plug that was seated. Pull tubing out of liner hanger then wash out hole.
1511122291 2015-07-08 Storage Well No Violations The surface owner, Dave Baker, contacted the inspector regarding a large area of dead vegetation around the well and fluid leaking from the well. The landowner was on site during the inspection. Dead vegetation starts on the hill just above the pipeline tie in and proceeds down hill and down the ditch along the access road from that point. There was a small puddle of standing water near the pipeline that had an orange color and a sheen on top. The water tested 0.8 on a high range Quantab which corresponds to less than 291 ppm Chloride. The sheen broke up into platy structures when disturbed indicating organic material and not oil. Another puddle at the access road below the well tested 0 for chlorides on a high range Quantab strip. A third puddle was tested in the access road ditch below the well location and indicated no detectable Chloride. Columbia Gas was contacted and advised to check the well for problems. The valve on the surface casing annulus was closed and a bull plug was in the valve. No leaks were observed from the well or pipeline. The source of the fluid that killed the vegetation around the well is not clear at this time. The hill around the well is wet indicating a seep, however the water tested at the location is not salty. Dave Baker can be reached at 419-368-7756. Terry VanSickle of Columbia checked the well and did not find an issue there. He stated that a vegetation management contractor was working on the pipeline right of way and that they applied herbicide that caused the dead vegetation.

The well is equipped with a flanged wellhead with 1-master valve and 1-wing valve. Well identification is clearly displayed. The well is located ~260 from its RBDMS location. A map showing the location discrepancy, as well as a request for the location to be updated in RBDMS was sent to Aaron Lantz (DOGRM Columbus). No other problems were noted at the time of the inspection.
