0951577019 2018-03-15 Production Wells No Violations There is 4.5 inch casing in the well with a tubing hangar type wellhead installed on it. No rods or tubing are visible in the open casing head. There is flowline at the location as well as an open pit. A request to inspect the well was received from Ben Harpster of the Orphan program following an inquiry by the landowner, Dan Stapp. There are 2-100 barrel tanks at the entry and a 50 barrel tank near the well. Following Chiefs Order 88-01 and enforcement case 89-A16, the well was referred for orphan plugging. Harold Moravy of the Division referenced an orphan file for the well in 2005. Ben stated he would check the Orphan records for the file and update the database.
1111440513 2005-03-21 Orphan No Violations Sherry Young request. Berea well with 4.5 casing. 2- 100 barrel and 1-50 barrel tank. 2 steel line will have to be removed. Road across creek will have to install culverts. Land is marsh and will have to be plugged during dry season. Many small trees to be removed. Open well. Roger Isch court case follow up. Called Alaric McConnell, son of Miller to advise of I&O program. He requested information. I am sending I&O packet with application. See I&O file for remainder of well reports.
1428524915 2015-04-08 Production Wells No Violations The well is stripped. There is open 6 inch casing at ground level. There is an old pit near the well. Old steel flow line is present and an electric pole is down at the well site. There is no power from the main disconnect. 2-100 Bbl tanks are at the lane entry and a 50 Bbl tank is near the well. The well was referred to the orphan program. Bond was forfeited years ago. Harold Moravy inspected the location in 2005. The well has not yet been plugged and is not capable of production.
