1951555 2011-02-17 No Violations Noted Site under construction. Well staked. Site ID sign& ES plan present. Observed mud on SR 217, Larry of North East Energy assured dept broom truck was on the way to clean road. Operator plans to encapsulate drill cuttings but based on Seasonal High Ground Water table, 18-36in, operator will be unable to do so and/or the pit must be constructed 20in above the SHGWT. One unlined pit located onsite, it appears that water was flowing in to pit pit bottom had water present. Based onobservation and SHGWT operator will either have to utilize an above grade pit or tanks. For the time being Larry of NEE took it upon him self to stop construction of pit and pump water out (filter thru bag) based on observation prior to dept¿s insp. It was also noted that during construction, a field drain was hit,cracked,&repaired. Ffield drains may be a slight contributor to water in pit, but SHGWT is <20in minimum. Spoke with Mike Buhite 2-18-11 plan for onsite mtg. Accompanied by WQS Marc Maestra. ES controls present, addtl FF reccommended near soil pile. Monitor all BMP's

2007752 2011-10-04 No Violations Noted Drilling complete. Pumpjack operating. Site graded but no seed or mulch. No tanks. Talked to property owner onsite.
2012897 2011-10-19 No Violations Noted Site visit was part of routine inspection. April Weiland WQS was present during inspection.

Site had been recently seeded and mulched. No growth had yet to take place. BMPs were not maintained. Several areas of silt fence were either overtopped with soil or completely knocked down. No erosion was leaving the site or entering the creek. Pump jack oil was stored in multiple places around the site. These should be contained in a manner that will limit the possibility of a pollution event, either accidental or through an act of a third party. No violations noted at this time.
