0045392794 2013-09-18 UrbanProduction Wells No Violations Follow up to Compliance Notice #1378835811. My inspection found that the weeds have been cut down in and around the tank battery and pumping unit. The belt guard on the pumping unit has been reinstalled. The gate was locked but the wellhead area still needs to be cleaned up of oil as well as the connections and fittings on the wellhead. The tank battery was cleaned up but the burned area outside the west side of the dike still requires reclamation and seeding and the access gate still requires a lock. Enervest is progressing with addressing the issues outlined in the compliance notice.
1378835811 2013-09-05 UrbanProduction Wells Violations Noted This Compliance Notice is being issued after following up on Complaint #4402. My follow up inspection today to inspection #1922440309 (8/8/13) found that the leaking 210 barrel tank has been removed. Nothing else I requested be done has been addressed. I e-mailed Enervest a list of what needed to be done at the site on 8/12/13. It has been almost a month and the following is a list of required actions necessary to come into compliance: 1) Enervest must reclaim and reseed the burned area outside the dike. 2) Address general housekeeping at the location to take care of the weeds at the pumping unit and tank battery. 3) Both gates need to be securely locked at the wellhead and tank battery as required by H.B. 278. 4) Connections/fittings at the wellhead are leaking and need to be replaced or fixed, and cleaned up around the fittings and on the ground. 5) The connection/fitting at the poly tank also needs to be fixed and the ground cleaned up below the valve. 6) They need to re-install the belt guard on the pumping unit. 7) Make sure the pumpers keep the lids on the tanks secured.
1380907863 2013-10-04 UrbanProduction Wells No Violations Follow up to Compliance Notice #1378835811. My inspection found that all off the requested compliance issues at this well and tank battery have been addressed. Both gates are securely locked, the weeds have been removed, the belt guard has been reinstalled on the pumping unit, the wellhead has been cleaned up, and the burned area on the west side of the tank battery has been cleaned up and seeded. This compliance notice is closed.
1922440309 2013-08-08 UrbanProduction Wells No Violations I inspected this well in regards to complaint #4402 about a leaking tank, a dead spot in the grass, and general poor condition of the well site. During my inspection, I found a diked and fenced tank battery containing a 210 barrel tank, 1-100 barrel tank, a 50 barrel poly tank, and a horizontal separator. The 210 barrel tank appears to have a leak in the heater tube and the connections at the base of the poly tank are leaking and need repaired. The 210 barrel tank was empty. The proper ID was posted on the 100 barrel tank. The lid of the poly tank was lying on the ground and the top of the tank was open. Tall weeds were pervasive inside and around the perimeter of the tank battery and along the first 75 of lease road as well. On the west side of the tank battery, it was obvious that brine had leaked under the dike and scalded the grass in an area of approximately 50 x 8. Several areas of the fence were sagging but not falling down and the access gate to the tank battery was not locked. I inspected the wellhead which was operating with an electric pumping unit and found that it was fenced but the gate was not locked. The pad lock was hanging on the fence and I locked it following my inspection. The pumping unit was not diked and the area inside the fence was overgrown with weeds. The belt guard of the pumping unit was lying on the ground to the west side of the unit. I found numerous small leaks at the wellhead at tubing connections and connections coming off the 4.5 casing. There was a small amount of product on the ground below two of the leaks that appeared to be accumulating for some time. I talked to Woody (Enervest) about the location and he said he would have the leaking tank removed and reclaim the burned area. I e-mailed him a list of what needed to be done at the site on 8/12/13. I asked them to reclaim and reseed the burned area outside the dike. I asked them to address general housekeeping needs to take care of the weeds at the pumpin
