0006384575 2015-02-25 Production Wells No Violations On the day of my inspection I witnessed that the production equipment included a 100bbl tank, a horizontal separator, a drip, a 2 inch sales meter with a .75 inch orifice and a chart recorder within an earthen dike. The well was equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing, a plunger lift system and domestic gas service with valves open on the tubing and 1 of 2 open on the casing supplying the domestic gas and motor valve. Identification was present and production records were current.

2080631722 2016-07-26 Production Wells No Violations On the day of my inspection I witnessed that the production equipment included a 100 barrel tank, a horizontal separator, a drip, a 2 inch sales meter with a .75 inch orifice and a chart recorder within an earthen dike. The burner tube on the 100 barrel tank has a drip which was field testing revealing a pH of 6 and a chloride level greater than 6215 parts per million. An area of 1 foot by 2 feet near the tank was contaminated. I placed call to Enervest well tender Zach Chaney and stated my findings. Mr. Chaney met me at the location and reviewed my findings. A water truck arrived, pulled remaining fluids from the tank and a plug was installed in the burner tube of the tank until it could be replaced.
