The wellhead is equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing. The tubing and casing valves are in the open position. A pressure gauge tied into the production line from the tubing reads 40 pounds per square inch. Production records are current.
1470926232 2016-08-11 Production Wells No Violations On the date of my inspection I found the tank battery to include 2 properly identified 100 barrel tanks, a concrete brine vault, horizontal separator, and a production meter box with a current chart, 2 drips, and a domestic diaphragm meter, within an earthen dike. Ponded water around the base of the separator with high chloride levels (Quantab >1,500 ppm). An area approximately 3 feet wide by 15 feet long has brine staining leading to the ponded water. Slow leak from the bottom valve of the tank. The well is equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing. The tubing and casing valves are in the open position. Production records end in 2014. Pictures were taken. I spoke with Jim Posey with Samurai Corp about clean-up. He indicated that he would repair or replace the leaking tank and fix remove the contaminated ground between the tank and separator.
1472478221 2016-08-29 Production Wells No Violations I am on site today to follow up on my findings from a previous inspection on 8/11/16. The operator has removed contamination surrounding the well head and placed new gravel on the area. A noticeable gas leak where the casing is tied into a 0.25 inch metal line can be heard. I notified the operator who indicated he would fix it today. Contamination within the tank berm between the tank and separator has been removed and new gravel has been placed on that area as well. The leaking area has not been repaired but the tank has been drained and is shut off from the system. The operator indicated he would repair it soon. The area immediately surrounding the horizontal separator within the tank berm is still saturated and the equipment is heavily rusted and deteriorated.
1526044749 2018-05-11 Production Wells No Violations On the day of my inspection I found this well equipped with an operating walking beam pumping unit. Well has 4.5 inch diameter casing, 1.9 inch diameter tubing, and rods. Production equipment includes two 100 barrel steel tanks, horizontal seperator, drips, domestic meter and a concrete brine vault. Vault is 18 inches from full. Gravel within containment appears to have brine staining from tank lines. Evidently a valve was recently replaced and a line was patched with a band clamp. Contamination will need cleaned up. Production records are up to date.
1530103871 2018-06-27 Production Wells No Violations On the day of my inspection I witnessed that Jim Posey was onsite with a service rig. They pulled the rods from the well and changed out the pump downhole. They were running the pump and rods back in during my inspection. Rods are laid out over plastic to keep contamination off of the ground. Jim said they will finish up and clean up today.

The wellhead is equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing. The tubing and casing valves are in the open position. A pressure gauge tied into the tubing line reads 25 pounds per square inch. Currently there is a metal chain from the frame on the pumping unit to the wellhead. This chain was placed to prevent the pumping unit from walking away from the wellhead (according to Jim Posey with Samurai). There are a few rods on the ground hear the wellhead and 1 within the production dike. Production records are current.
2040208139 2013-07-16 Production Wells No Violations I was on location to do a status check on this well. The entire tank battery and the production equipment has been repainted and a new sign with updated information has been posted. The well is located near the fairway of the golf course. There are no violations at this time.
