2629514 Primary Facility 2017-08-23 Routine/Complete Inspection No Violations Noted Routine inspection of of ARD Operating LLC's COP TRACT 653 Pads B&C on August 24, 2017. No one was onsite during my inspection. This ESCGP (ESG29-035-16-0001) covers both Marcellus well pads B&C.

Pad B: There are two active Marcellus wells on this pad (035-21162 and 035-21174). Each well is tagged with the permit number. Both cellars are full of water. Each well is plumbed to its own production unit. In addition there are (3) 400 BBL produced fluids tanks located in secondary containment. No water was observed within the containment.

Pad C: There are three active Marcellus wells on this pad (035-21161, 035-21264, and 035-21273). Each well is tagged with the permit number. All cellars were full of water. Each well is plumbed to its own production unit. In addition there are (4) 400 BBL produced fluids tanks stationed in secondary containment. Just a trace of water was observed within the containment.

Vegetation exceeds 70% around both well pads and both pads appear stable. No PCSMs were observed at eithe

2806204 Primary Facility 2018-11-20 Routine/Complete Inspection No Violations Noted On November 5, 2018, Department representative Brad Kline conducted a routine site inspection at ARD Operating , LLC. COP Tract 653 Pad B. Temporary E&S controls and site restoration could not be evaluated due to snow cover. ESG29-035-16-0001 covers both Pad B and Pad C in the permit.

There are a total of 2 producing wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 2 GPU’s and 2 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment. The steel production fluid tanks had some minor corrosion occurring (Photo 3). The Department recommends monitoring the condition of the tanks.
