1859349582 2018-03-15 Production Wells No Violations On the day of my inspection I found that the well coordinates were plotting in an open area, along an access road that connected with the access road for the number 1 well. I found some old sections of steel and plastic line around the general area. There was limestone gravel spread around the center of the open area and up the access road toward the number 1 well. I was with Twin Oaks pumper Steve Davis to confirm the location and status of this well. We did not find any wellhead components or casing in the area. I also did not find any indications that there was a leaking plugged well. The current well status is listed as producing and there has been production reported by Twin Oaks as recent as 2012. I found through my conversations with Steve and Glenn Lovell that they both believed the number 1 well was the number 6, and they were reporting the production from the number 1 under this permit number. I also found an inspection report from Rocky King claiming this well was in production in 2006, but when I plotted the coordinates that Rocky had recorded they plotted on the number 1 well location. I searched the well records and found the following chronology of the Heffelfinger #6, permit 0739: spud 5/18/1955, completed 6/15/1955, plugged 1/15/1970 by Preston Oil; then Twin Oaks applied to reopen the well in 1979, with a spud date of 7/5/1980, completed 8/3/1980. At this point the records are questionable, and the scanned copies in RBDMS are barely legible. I did not find an application for, or an issued plugging permit for this well after 1980. I did find a copy of a plug report from the Ohio Division of Mines dated 6/4/1986, which states that the well was plugged and abandoned on 6/28/1985. The copy of the report was in poor condition and I could not clearly read the plugging methods or materials used. Finally, I found two copies of final restoration reports.
