1356703052 2012-10-25 Today I witnessed the cementing of the 8 surface casing for David R. Hill Inc. on Kilbarger rig 1. Universal Well Services was contracted to cement the casing. Circulation was established. Good cement returns were observed and cement stayed at the surface. No cement basket was used on the casing.
1358273694 2012-11-04 On 10/31/2012 John Suhar with David R. Hill Inc. called to inform me that the well was going crooked once they began drilling directionally. The plan was to set a cement plug through drill pipe to use as a kick off plug just below the surface casing. Universal Well Services was on location on 11/1/2012 to set a cement plug. The first plug was a 650 foot plug from 2300' to 1650' set with 50/50 poz. cement. The second plug was a 350 foot plug from 1390' to 1040' set with 50/50 poz. cement. The third plug was a 540 foot plug from 1040' to 500' set with Class A cement. All plugs were witnessed. Kilbarger waited 24 hours before they went back to drilling. Cement tickets are attached with surface tickets.
1382123228 2013-10-11 This inspection was conducted as a preliminary restoration status check. The well was equipped with a rabbit wellhead. The well produces into (2) 100 barrel tanks inside an earthen dike. A gas separator sits outside of the tank battery. Each tank has a charcoal filter. Identification posted at the tank battery. The area has been restored. Restoration meets ORC 1509.072.
2002942919 2012-11-04 On 11/4/2012 I received a phone call from John Suhar with David R. Hill. He informed me that the well was going crooked again. Universal Well Services was contracted to set another cement kick off plug. I was not able to witness this job. Cement tickets are attached with the surface tickets.
-2058462035 2013-10-11 I inspected this location in regards to a complaint about a gas odor. The complainant, Mrs. Jobeth Fruechnicht, claims that her house was overtaken with a strong gas odor and was making her children ill. Mrs. Fruechnicht's house is located just to the south of the well location. I inspected the location. My gas detector did not pick up any methane during inspection. I called and left a message for David R. Hill Inc. in regards to the complaint. During the inspection the pumper pulled in. He informed me what was happening was the separator was dumping gas in the tanks as the line pressure was too high to sell gas into and this was the reason for the gas odor. He explained that they were working on correcting the problem. I stopped by the complainants house and informed her that the company is working on the problem and should not be an issue in the future.
2124071477 2012-10-23 I witnessed the cementing of the conductor casing today for David R. Hill Inc. on Kilbarger rig 1. Universal Well Service was on location to cement the casing. Circulation was established. Good cement returns were observed and stayed at the surface. No cement basket was used.
399616570 2012-11-06 I inspected drilling operations today. Kilbarger tripping in drill pipe during inspection. No violations.
-890782552 2013-10-16 I inspected this location today. Dan Walker with Dave Hill Inc. was on location. During inspection the company was installing a combuster to flare gas being dumped off from the separator. The company is correcting a problem that was affecting a house south of the location.
